Van Driver/Activities Services Assistant
We are seeking hardworking, dedicated individuals to provide safe, courteous, and on schedule transportation for our clients in conjunction with the implementation of the activity program in the facility. The activity program shall meet the needs and interest of all patients.
Wayne Nursing & Rehabilitation Center is a 60-bed care community providing skilled nursing, short-term rehabilitation to home services, and long-term care.
Responsibilities:

1. Schedule and track appointments in system.

2. Transport patients to scheduled appointments or outings as needed.

3. Maintain weekly appointment schedule and maintain appointment changes.
-
4. Maintain the Vehicle Usage Log.

5. Assist with Vehicle Service schedule.

6. Maintain Vehicle Inspection Log.
-
7. Ensure van is cleaned after each use.

8. Train other van drivers as necessary.

9. Assist with the preparation of written monthly activities calendars.

10. Assist patients with transportation to activities, meal services, and special functions.

11. Record accurate documentation in electronic medical records.
Requirements:

- High School diploma or GED equivalent.

- Valid Driver’s License and insurability
-
- CPR certification (or acquire within first 90 days of hire).

- When applicable, a Food Handler’s Permit (or acquire within first 6 months of hire).

- Completion of company-approved driving course.

- Basic computer skills (preferred).
