Registered Nurse - Case Manager

Position Summary:

The Registered Nurse is expected to provide comprehensive care to patients through the activities of assessment, planning and provision of direct nursing care within the policies and standards of the Agency. The Staff Nurse/Case Manager position reports to Clinical Manager and is a non-exempt position.

Required Experience for Registered Nurse:

  • Currently licensed registered nurse in the State of Michigan.
  • Minimum of 1 year of nursing in a home care setting is preferred.
  • CPR certification preferred.
  • Excellent communications skills.
  • Compliance with accepted professional standards and practices.
  • Self-directed with the ability to work with little supervision
  • Avid team player and must understand a team approach.
  • Consistently maintains a positive attitude which promotes team and optimal performance

Accountability for Registered Nurse:

  • Accountable for an assigned caseload of patients to a developed plan of care.
  • Honors patient’s rights. Adheres to privacy and security standards provided by all federal, state and local authorities of the Health Insurance Portability and Accountability Act (HIPAA) and other applicable state laws.
  • Provides care, treatment and services in accordance with professional standards of practice, practice guidelines, law, and regulations and established Plan of Care
  • Develops and implements a comprehensive, goal focused, patient centered care plan for each patient and makes updates in accordance with written orders from the physician ensuring patient collaboration and understanding.
  • Makes planned home visits to the patient for assessment, instruction, supervision, and provision of nursing treatments per the Plan of Care.
  • Makes referrals to other services as indicated.
  • Maintains an open communication process with the patient, representative’s, caregivers, physicians and other health care providers to report the patient’s response to home care, any changes in status, or possible need for additional services.

Responsibility for Registered Nurse:

  • Is responsible for the observation, assessment, nursing diagnosis, counsel, care and health teaching of the ill, injured or infirm, maintenance of health, promotion and prevention of his or her patient’s illness.
  • Administers medications and treatments as prescribed by the patient’s physician or podiatrist.
  • Acts as the coordinator of the health care team in order to maintain the proper linkage for interdisciplinary case conferencing to promote a quality continuum of care.
  • Prepares and maintains clinical documentation according to Agency policies and acceptable nursing standards.
  • Provides instruction/guidelines to home health aides providing health and personal care service, including ongoing supervision and evaluation of the services delivered.
  • Coordinates therapists, home health aides, and others who provide services to each patient to form an interdisciplinary team to ensure consistent and appropriate care. Teaches the patient and those in the home who nurse the patient how his/her care is to be given.
  • Conducts OASIS assessments at appropriate time points.
  • Provides realistic patient/nursing goals and expected outcomes.
  • Evaluates the effectiveness and outcome of care.
  • Complete visits and submit a complete documentation efficiently. Cooperate with Quality Assurance record reviewer and perform needed corrections of records.
  • Participates in the Outcome Base Quality Improvement and Management.
  • Initiates preventative and rehabilitation nursing.
  • Supervises and evaluates the patient’s care on an on-going basis.
  • Implements infection control and safety precautions, and complies with infection control policies and procedures
  • Honors patient rights. Informs the patient of his or her rights prior to beginning care, treatment, or services or during the initial evaluation visit before beginning care, treatment, or services.
  • Immediately Reports cases of possible abuse, neglect, and exploitation to the Administrator, Alternate Administrator, Clinical Manager or Compliance officer.
  • Plans for the patient’s discharge from service and coordinates discharge planning with the physician.
  • Informs the Clinical Manager of patient services provided and needed and any changes in the Plan of Care.
  • Participates in orientation, in-service programs, staff meetings, and case conferences.
  • Interprets Agency services and policies to patients, families, physicians.
  • Maintains a working knowledge of third-party reimbursement in relation to home health patient benefits.
  • Recognizes legal responsibilities involved in patient care, respects patient rights, and property.
  • Acts as a source of information and provides mentoring, and sets an example for less experienced staff members.
  • Contacts the Clinical Manager to inform him/her of alternate schedule for patient visits during vacation, sick days or personal days off. Reports any schedule change or inability to complete assignment as scheduled.
  • Compliance with all applicable laws, regulatory requirements, standards of practice, accreditation standards and policies and procedures.
  • Accepts changes/additions to schedule during staffing difficulties when necessary. Accepts assignment in time of emergency, regardless of any location and/or any day of the week be it weekend/holidays.
  • Organizes daily visit schedule to minimize travel time and mileage. Maintains productivity according to work status.
  • Maintains OSHA and Infection control per policy.
  • Performs other duties as assigned.

Physical Demands:

  • Must be able to meet the physical demands of providing nursing care, ambulating patients, transferring patients, assisting with exercises, etc. without restrictions.
  • Must be able to transfer patients as needed according to proper techniques of body mechanics to promote patient and staff safety and well-being without restrictions.
  • While performing the responsibilities of this job, the employee is regularly required to talk and hear. The employee frequently is required to stand, walk, sit, lift, use hands to handle or feel, reach with hands and arms, and stoop, kneel, crouch or crawl with no restrictions. Ability to wear Personal Protective Equipment (PPE).

Unusual Demands:

  • The employee may occasionally be exposed to wet and/or humid conditions, fumes or airborne particles. The noise level in the work environment is usually moderate.
