Ascend Career Services is partnering with the Arc Alliance to hire a Supports Coordinator / Case Manager for the Montgomery County, Pennsylvania area. Key Responsibilities:

  • Responsible for direct provision of case management services and coordinating all direct services
  • Interview individuals and others as appropriate
  • Responsible for the development of an initial Individual Support Plan (ISP). Plan and coordinate team meetings at least annually to conduct a comprehensive review of the ISP
  • Coordinates and assures delivery of direct services
  • Monitor participant and/or family satisfaction with services. Arrange for modifications in services and service delivery.
  • Ensure that services are appropriately documented in HCSIS on the ISP
  • Attend and participate in community activities representing The Arc Advocacy Services; Participate/Contribute in agency fundraising activities
  • Other duties as assigned

Job Requirements:

  • Must have at least a bachelor’s degree from an accredited college or university with at least 12 college credits in sociology, social work, psychology, gerontology, criminal justice or other related social sciences
  • The candidate must have related work experience and have an understanding of local, state and the federal special education and social services assistance systems
  • Possess high quality organizational skills
  • Ability to adhere to strict time guidelines
  • Excellent communication/writing skills
  • Ability to understand and accept the needs and rights of others and to work with individuals who are physically challenged, emotionally troubled, or economically disadvantaged
