Position Summary: Provides assessment and intervention to assist clients/families to improve social and economic difficulties interfering with health and wellness through use of casework process and principles, strategies, and community resources. Employee coordinates, negotiates, procures and manages the care of inpatient Rehabilitation patients. Employee facilitates quality clinical outcomes, promotes patient satisfaction, assist in management of length of stay and oversees the discharge planning process.
Essential Functions and Responsibilities:

  • Employee determines nature of client's situation by interviewing client; assessing medical, psychological, emotional, and social information; making on-site visits and establishes course of action by exploring options; setting goals with client.
  • Employee obtains assistance for client by referring him/her to community resources; arranging for appointments; establishing rapport with other agencies.
  • Employee fosters client's action or adjustment by interpreting attitudes and patterns of behavior; explaining and pointing out new options.
  • Employee assists with appropriate discharge destination including family education, provides resource materials and contacts for potential discharge sites including list of Nursing home facilities, Assistive living, and community-based resources.
  • Employee recommends and/or secures health resources and clinical equipment required for patient and family to transition to discharge environment and arrange for post discharge medical follow up including home care, outpatient therapies, and physician visits as needed.
  • Employee performs individual and/or family therapy to assist to adjustment to recent rehabilitation diagnosis and loss.
  • Employee participates in interdisciplinary conferences, completes conference documentation and provides consultation for clinical staff. Employee reports progress and needs for patient to team and family throughout the patient’s rehabilitation stay.
  • Employee may oversee fund requests for specific patient needs post discharge including home modifications and durable medical equipment not covered by insurance.
  • Employee completes Medical Record and department documentation including barriers to goal attainment, and client/family response to intervention.
  • Employee monitors planned actions by periodic follow-up with team, physicians as well as patients and family.
  • Employee maintains effective operations by following policies and procedures; participating in quality reviews; reporting needed changes.
  • Employee maintains professional and technical knowledge by attending educational workshops; reviewing professional publications; establishing personal networks; participating in professional societies. May develop and conduct in-service training programs.
  • Employee complies with federal, state, and local legal and certification requirements by studying existing and new legislation; anticipating future legislation; enforcing adherence to requirements; advising management on needed actions.
  • Employee assists in quality control and performance improvement activities specific to the rehabilitation unit.
  • Employee contributes to performance feedback, hiring decisions and orients and/or trains new staff and may assist across the Rehabilitation network.
  • Employee evaluates and tests new procedures/processes.
  • Employee will monitor and assist with workflow/distribution of work.

Employee will assist in assuring compliance with all regulatory and other agency requirements, laws, and statutes pertaining to the operation of the rehabilitation department including but not limited to discharge destination and 60% compliance rule.
Required:

  • Masters degree in Social Work from accredited educational institution
  • State of Michigan licensure as a Licensed Master’s Social Worker (LMSW)

Preferred:

  • Prior rehabilitation experience preferred
  • Basic Life Support (BLS) certification as a Healthcare Provider by the American Heart Association or equivalent through the Military Training network (MTN)

Knowledge, Skills, and Abilities:

  • Ability to manage patients with different types of personalities.
  • Current knowledge of treatment theories, practices, and techniques per discipline.
  • Demonstrated ability to read and interpret documents such as safety rules, operating and maintenance instructions, and policy and procedures manuals.
  • Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists.
  • Knowledge and skill with common office software applications.
  • Compliance with the Code of Ethics and Guide for Professional Conduct.
  • Ability to work independently and provide sound judgement in interactions with the interdisciplinary team, patients, families, and payers.
  • Must be able to act calmly and effectively in busy/stressful environment.
  • Have strong organizational and time management skills
