Home Care Agency located in NW Washington DC is looking to hire RN/PT/OT/ST who are dedicated, team players and detailed oriented individuals who will be responsible for ensuring patient visit coordination, The RN creates OASIS initial evaluation and regularly reevaluates the patient’s nursing needs Initiates the plan of care and necessary revisions with recommendations and consultation with the Director of Clinical Services. Provides those services requiring substantial specialized nursing skills. Ensures proper wound care, urinary catheter care, insertion, IV therapy, Irrigation, suctioning, Tracheostomy care, G-Tube feed, and other complex nursing care as required by the physician's plan of care. Provide clinical home health care services as per the agency guidelines.

Completes certification/recertification orders and discharge summaries on time.
The RN shall conduct patient care conferences on patients assigned to his/her care.
Including participation in peer review and performance improvement, which includes to participate in utilization review of medical records as assigned. Lastly, giving total patient care as needed.

PT/OT/ST DUTIES:
OASIS evaluation or Initial Physical/Occupational therapy evaluations and subsequent therapeutics needs and providing services in accordance with the plan of care. Initiates the care plan with necessary revisions and implements appropriate preventive and rehabilitative therapeutic procedures. The clinician prepares clinical and progress notes for each patient visit and summaries of care conferences on his/her patients on time as per Agency policy which includes Coordinating clinical services and informing personnel of changes in the condition and needs of the patient.
Participates in and presents in-service programs. Understands and adheres to established Agency policies and procedures, including notifying physicians of patient needs and conditions changes. Completes certification/recertification orders and discharge summaries on time.
