Connected Home Health & Hospice is hiring a Home Health RN (Registered Nurse) to serve on the west side of the Greater Portland, Oregon area (Beaverton, Hillsboro, Forest Frove).

RN Essential Duties

• Provide one-on-one skilled nursing care and treatment to patients in their home
• Development and implementation of Plan of Care
• Provide Skilled observation, assessment and evaluation
• Patient and family/caregiver support and education
• Cohesive and collaborative care with LPN and Therapy Team

Requirements and Qualifications

• Registered Nurse (RN) licensure in Oregon
• 2+ Years RN experience is preferred
• Ability to work independently and demonstrate critical thinking
• Current BLS certification from AHA
• Be a licensed driver with an insured automobile in good working order
• Ability to comply with the State COVID Vaccination Mandate
