Medical Director, Transitions of Care (Senior Physician)

LOCATION: Olive View-UCLA Medical Center, 14445 Olive View Drive, Sylmar, CA 91342 OVMC is located in the city of Sylmar, which is a part of the San Fernando Valley. The population, as of 2009, is approximately 90,000 residents. The city’s neighboring cities are San Fernando and Santa Clarita. The Olive View hospital has been part of the neighborhood since 1920, when it initially served as a tuberculosis (TB) sanatorium.

GENERAL SUMMARY: Olive View-UCLA Medical Center (OVMC), a 337-bed public teaching hospital affiliated with the David Geffen School of Medicine at UCLA, is seeking a Senior Physician (Internist or Hospitalist) to serve as Medical Director, Transitions of Care. Individual will receive a health sciences academic appointment in the UCLA Department of Medicine, and will be required to contribute to the academic mission (research, education, and/or develop clinical programs). Position is 50% administrative and 50% clinical.

JOB DUTIES/RESPONSIBILITIES:

• Build workstreams and clinical pathways to reduce unnecessary or avoidable Emergency Room and Inpatient utilization. Role will address three areas:
• - Reduction in unnecessary ER visits
• - Reduction in unnecessary admissions
• - Reduction in unnecessary length of stay / hospital bed days
• Work collaboratively with hospital leadership, including the Director of Inpatient Services, the Director of Utilization Management and Social Services, department chairs, and division chiefs to realize the goals outlined above
• Build upon current transitions of care pathways throughout DHS, including the “Next Day Clinic” and “Safer at Home” Programs, to ensure acuity of illness is addressed in the right clinical setting
• Allocate 50% of time to clinical duties, including primary care or hospitalist, depending on applicant qualifications, institutional need, and applicant preference
• Facility with data tools and quality improvement processes

REQUIRED QUALIFICATIONS:

• Graduated from an accredited school of medicine
• Completed an ACGME-accredited General Internal Medicine Program
• Board-certified in Internal Medicine
• Innovative and creative thinker

DESIRABLE QUALIFICATIONS:

• Experience working in a government or non-profit environment serving a diverse, medically-underserved population
• Experience in management/leadership roles
• Formal training in quality improvement and/or LEAN
• Excellent written and verbal communication skills
• Ability to work in a collaborative/team-oriented environment

Interested candidates must currently hold a permanent County of Los Angeles payroll title of Senior Physician to be considered for a lateral transfer, or be reachable on a DHS certification list to be considered for a promotion or as a new hire.
