Trustpoint Hospital is looking for someone with experience in physical medicine and inpatient rehabilitation for our Intake Department. Must be a RN or Licensed Therapist.

TrustPoint Hospital offers the only acute, hospital-based rehabilitation in Middle Tennessee. Our purpose is to help people recovering from illness or injury attain the highest possible quality of life. Our rehabilitation center has a large therapy gym with state-of-the-art equipment and an "activities of daily living area" which includes a kitchen and laundry area. Patients receive focused care and comprehensive support they need throughout their journey to improved health. This unit offers occupational, physical, speech, and respiratory therapy. Other services like IV therapy and wound care are also provided.

PURPOSE STATEMENT:

This position directly receives referral calls and/or walk in referrals, assessing patients for admission status, on-site evaluations, completing insurance pre-certification and admission documentation if applicable.

ESSENTIAL FUNCTIONS:

• Facilitate intake, admission and utilization review process for incoming patients.
• Perform insurance benefit verification's, disseminating the information gathered to patient, their families and appropriate internal staff.
• Provide accurate and ongoing assessment of patient’s status in the admission and utilization process.
• Respond to inquiries about the facility within facility policy time frames.
• Schedule/complete pre-admission assessments and communicate recommendations to patient or their family.
• Collaborate with other facility medical personnel to ensure appropriate recommendations and admissions.
• Maintains up-to-date statistical data in computer information system within established time frames.
• Ensures completion of all referral documentation and maintains filing systems.
• Maintains, posts, and distributes an accurate census and referral information to administration hospital staff on a daily basis.
• Maintains systems and processes for timely data-entry of pre-admit, admission, and registration of information for inpatients and outpatients.
• Compiles daily and monthly statistics for admission, discharges, referrals and denials. Prepares statistics for corporate report as designated. Compiles and distributes weekly admissions/discharges list. Monitors pending list daily.
• Pre- Admit all patients into appropriate EMR/Billing system - obtain correct demographic, clinical, and insurance information. Contact patient's insurance to confirm and determine amount of coverage and benefits, deductible due, length of stay and if pre-certification is required, in order to complete Insurance Benefit Verification form.
• Obtain Pre-certification for inpatient rehabilitation stay and document necessary information for case management to follow up once patient admits.
• Answers telephone promptly and courteously, providing information and completing all referral/intake forms as appropriate for each call.
• Other duties as assigned.
