Job Summary:
The Home Health Aide position is a highly rewarding position that helps clients remain in their homes by improving their quality of life and contributing to a safer environment. The Home Health Aide assists individuals with personal care and light housekeeping, as well as emotional support. The Aide administers personal care and assistance with activities of daily living for patients in the home setting under physician’s orders, assignments, and supervision provided by the Supportive Care Manager. The Home Health Aide reports to the Supportive Care Manager and second to the Nurse Manager.

Essential Qualifications: Current state registration as a Licensed Nurses Aide with 1 - 5 years of experience in an acute, intermediate or skilled nursing unit Valid driver’s license, good driving record, and certificate of auto insurance are required. The Home Health Aide must be highly self-motivated, non-judgmental, flexible, possess excellent communication skills, and be able to work independently. Preservation of confidentiality is mandatory.

Hospice care includes a certified hospice nursing assessment, 24/7 clinical support, in-home provider visits for pain and symptom management, medical supplies, equipment and hospice medications, volunteer support and companionship, social and spiritual support, and bereavement and grief support.
