We are seeking a Healthcare Navigator to work at Master Center for Addiction Medicine! We are a private, outpatient addiction medicine treatment center headquartered in Glen Allen, Virginia. We have additional offices in Hampton and Gloucester and we are expanding to provide services to other communities in need, particularly rural communities, where addiction treatment may be difficult to access. Our approach to addiction is comprehensive, evidence-based and individualized, drawing on every tool that has been proven effective: medical treatment, education, psychiatry, professional counseling, testing/monitoring and peer support services. Our aim is not just to treat patients but to heal them – by helping each one find his or her way to lifelong recovery.

The Healthcare Navigator works closely with the Directors of Operations, Client Development, and Care Quality and Coordination. The Healthcare Navigator is committed to removing patients’ barriers to care. Responsibilities include maintaining accurate community resources, identify barriers to treatment, assisting uninsured patients with Medicaid enrollment, onboard new patients, and ensure timely coordination of care.

The job requires keen attention to detail and the ability to multitask combined with patience, empathy, and respect for the callers. This is a position that will involve a significant amount of time one-on-one with patients, and it requires the ability to communicate effectively over the phone with patients and the ability to communicate effectively with other team members regarding patient care. This is a fast-paced role and you will spend a great deal of time on the phone and computer. The ideal candidate will know how to effectively multi-task while staying engaged with patients.

We do candidate assessment batteries. You are required to complete the competencies. If you do not complete the assessments, the hiring manager will not schedule an interview with you after the initial phone interview.

DUTIES AND RESPONSIBILITIES:

Care Coordination

  • Meet with the small care teams bi-weekly to discuss patient care issues and needs to develop a robust community resource list accessible to all staff.
  • Help identify barriers to treatment, including social determinants of health.
  • Work alongside outreach team to develop community resource partnerships.
  • Assist the Director of Care Quality and Coordination by completing record requests from referring providers.
  • Assist the Director of Care Quality and Coordination with patients who have completed a higher level of care and wish to return to Master Center.
  • Be flexible and adaptable in response to changing client and health care needs.

Patient Assistance

  • Help locate, develop, or obtain needed services, resources, and appropriate public health benefit programs for patients.
  • Work in conjunction with patient intake services to onboard new patients.
  • Work with uninsured patients directly to assist in Medicaid enrollment if requested.
  • Verify insurance and assist patients with COB challenges.
  • Performs customer service activities and respond to patient inquires in a timely manner.
  • Maintains HIPAA Compliance by protecting Patient Health Information according to established policies and procedures.

Other Responsibilities:

  • Serve as liaison between intake, outreach, and care coordination departments.
  • Always represents Master Center in a positive and professional manner.
  • Adhere to departmental policies and procedures.
  • Maintain training documents, forms, and job aids based on operational changes.

Other projects and duties as assigned.

REQUIRED SKILLS:

  • Strong problem-solving skills, high attention to detail and an aptitude for learning.
  • Willingness to take initiative and identify opportunities for company growth.
  • Strong organizational and time management skills required.
  • Must provide exceptional customer service and professional communications to patients, colleagues, insurance entities, and professional staff.
  • Ability to communicate detailed and relevant data.
  • Ability to perform a variety of duties and work within a structured environment.
  • Work history in Behavioral Health highly preferred.

MINIMUM QUALIFICATIONS:

  • 3-5 years in healthcare and patient enrollment experience strongly preferred.
