PRIMARY FUNCTION : The Health Educator will assess enrolled clients for health education services, including individual and group sessions, to facilitate the development of necessary self-management skills for HIV+ individuals. Develops effective interpersonal relationships with clients and works collaboratively with the interdisciplinary care team to improve health outcomes. Utilizes internal and community resources to educate clients and form a care plan with specific health outcomes.

DUTIES AND RESPONSIBILITIES:

  • Screen referred client for health education needs. Develop and implement health education service plans.
  • Collaborate with the case manager in assessing client needs, developing services plans and participating in case conferences.
  • Develop health education curriculum and related activities; and facilitate group and individual health education sessions.
  • Ensure completion of documentation and tasks (intakes, assessments and service plans) by due date.
  • Conducts health education screenings using standardized tools on clients receiving case management under this contract
  • Provides individual health education sessions utilizing Motivational Interviewing and other evidenced based techniques.
  • Provides systems navigation for clients, including provision of reminder and follow-up calls, through accompaniment when necessary.
  • Facilitate support group activities and educational workshops affiliated with this contract.
  • Provides harm and risk reduction education as well as the importance of complying with medical regimens.
  • Coordinates & case conferences with relevant service providers (i.e. care managers) to ensure appropriate service provision and to minimize barriers to entry and retention in care.
  • Tracks/monitors client progress through a combination of written work, agency databases, and case conferences with the Care Team. Documents and maintains case records in agency database and completes all data entry in a timely fashion.
  • Participates in initial and ongoing trainings as necessary to maintain a level of knowledge related to HIV/AIDS and related co-morbidities. Always represents the Alliance with the highest level of professionalism and respect.
  • Performs other duties as required.

QUALIFICATIONS: Bachelor’s Degree in health or human services field required and one-year working with an underserved population through employment, volunteer work, and internships; Experience working in the field of HIV/AIDS, health education and chronic disease including Substance user, Mentally Ill, and Lesbian, Gay Bisexual and Transgender (LGBT). Fluency in Spanish required. Excellent written and verbal communication skills required. Excellent people skills with professional demeanor. Ability to handle multiple tasks and stressful environment. Ability to use Alliance’s and Grantor’s electronic data systems.

Job Type: Full-time

Pay: $37,000.00 - $42,000.00 per year
