The Position

• The Corporate Director of Care Coordination is responsible for the leadership and supervision to the staff within the Coordination of Care Departments at multiple locations.
• The Director will coordinate and support problem solving, as well as developing, implementing, and revising tools, processes, and systems to support the Coordination of Care Departments to optimize outcomes.
• Ability to work collaboratively with department staff, physicians, and healthcare professionals at all levels to achieve established goals, improve quality of outcomes, maintain, or exceed The Joint Commission standards and state mandates as they apply to the department operations.
• Focus on supporting the staff with difficult discharges and long length of stay patients by rounding with staff daily and assisting them with placement.
• This position will identify, recommend, and implement sustainable improvement in patient flow processes and operational efficiencies in the Emergency Department and Nursing Units.
• Bring teamwork and integration of services for the staff in the department and the customers’ served, while adhering to patient experience initiatives and champions customer services excellence.
• Minimum of seven years of experience in Case Management, discharge planning and/or utilization management with strong leadership skills. Multisite experience preferred.
• BSN degree required.
