Job Purpose & Position Overview

This position is responsible for providing case management services to patients utilizing the Crisis Response Program and the Winnebago Behavioral Health Program. Case Manager is responsible for assessing the physical and psychosocial needs for patients in crisis, needing support to stabilize and remain stable mentally and physically. The Case manager will develop and implement interventions to meet identified needs. This position monitors patient compliance with specific medication instructions and appointment guidelines and formulated treatment plans, educates patients and their families on their specific processes, and provides ongoing assessment, monitoring and re-evaluation of health related needs.

Specific Job Duties/Responsibilities:

  • Work with and coordinates care with the interdisciplinary team providing case management and education to the patient and their families
  • Develop individualized patient care plans for specific mental health and chronic health conditions.
  • Implement, evaluate and monitor care plans on a regular basis and make adjustments as needed
  • Develop methods for tracking patient compliance to treatment and care plans
  • Triage telephone calls from patients, family members or caregivers and referral health concerns to the appropriate staff
  • Assure patient confidentiality as defined by HIPPA and WCHS policies and procedures
  • Serve as a contact, advocate, and information/educational resource for the patient, family and community
  • Record and document patient care data necessary to maintain, coordinate and expedite required services according to established standards and procedures
  • Performs other assigned duties as needed

Qualifications/Skills:

  • Bachelor’s degree required, must possess nursing license (or other appropriate professional licensure) in Nebraska or other compact state
  • Proven experience as a case manager for two or more years
  • Must practice excellent communication skills and relate to easily to patients, families, and healthcare providers
  • Must have knowledge of Electronic Health Records (EHR) usage
  • Proficient with Microsoft Word, Excel, Outlook, and Power Point

Physical Requirements:

  • Normal to light office work which includes the ability to lift up to 10 lbs.
  • Regularly required to walk, stand, and sit for extended periods of time and use hands to operate computer keyboard, phone, and various office equipment.

Certificates, Licenses & Registrations:

  • All applicants will comply with 45 CFR 1301, Subpart D, Section 1301.31, C and D, which require all prospective employees to sign a declaration prior to employment regarding all arrests and convictions of child abuse or violent felonies and to comply with PL 101-630 and PL 101-637 regarding criminal records check.
