Summary:Care for patients in home setting. Provide assistance with activities of daily living.
Qualifications: * Certification by the State of Georgia required

• One year's experience in a geriatric and/or hospice setting is strongly preferred
• Must be comfortable working in hospice environment
• Must be able to be supervised, to follow directions, and to work on a team in the care of the patients.

Responsibilities: * Record accurately and frequently in the electronic health record

• Assist with personal hygiene (bathing, dressing, grooming and toileting) per patient choice and plan of care
• Assist patients with activities of daily living. Be alert to their comfort and needs. Answer their requests promptly.
• Keep case manager informed during shift about observations made concerning patients’ health, comfort, and safety.
• Take and report vital signs and weights as assigned.
• Keep patients’ rooms’ neat, bed made, bedside stand clean, nursing equipment clean (i.e. bedpans, urinals, enema equipment, etc.). Obtain replacements from office as needed.
• Keep patients’ clothing clean.
• Keep wheelchairs, walkers and commodes clean.
• Practice economy in the use of supplies. Use equipment carefully. Report equipment needing service or repair
• Encourage patients to participate in daily activities
• Stay up to date with online training as assigned monthly.
• Must be willing and able to drive to patient home (residential and facility) in the metro Atlanta area as assigned.

Supervisory Responsibility: No supervisory responsibility .

Physical Requirements: Ability to lift up to 30 pounds; ability to stand for 80% of the day.

Mental Requirements: Ability to focus on residents ongoing; ability to chart care.

Other Requirements: Must have reliable transportation.

*Paid incrementally over 1 year of employment.
