Case Manager (M-F 9am- 5pm) Job at Dimondale Adolescent Care Facility

Dimondale Adolescent Care Facility Torrance, CA 90505

Under the direction of the Head of Service, the Case Manager performs the following functions:

1. Providing crisis intervention, intake assessment, individual, and group rehabilitation services to adolescents with a history of mental illness.

2. Coordinating support services to assist clients with after care services.

3. Maintain a productivity standard of 70% of work schedule. (40 hours a week equals 28 hours per week, 173 hours a month equals 121.10 hours per month)

4. Plans and administers Individual and Group rehabilitation sessions as prescribed by the Integrated Core Practice Model. Provides a range of services including individual, and community based social rehab activity, collateral contacts, and case management services.

5. Assesses and monitors the condition of the client to evaluate success of the treatment plan and adapt interventions as needed.

6. Provides case management to include assessment of client/family needs, referral, links and support in accessing community services, psychiatrist, medical care, housing, rehabilitation programs and social activity groups

1. Monitors and maintains documentation of the client’s services within the client’s charts, ensuring compliance with Agency policies and procedures, Department of Mental Health contract and other related agencies’ standards and guidelines.

2. Develops and maintains case management skills through seminars, workshops, or other forms of training/education related to working with adolescents with a history of mental illness.

3. Attends and participates in the program, Agency staff meetings: and related community agency meetings as required/requested.

4. Arrives to weekly individual/group supervision prepared with cases to discuss and ready to receive feedback and direction from supervisor.

Qualifications:

1. Must be at least 21 years old.

2. Must be physically and mentally fit to perform all duties.

3. Must have a criminal record clearance for DOJ, FBI and child abuse index clearance.

4. Ability to interact and communicate both verbally and in writing to clients, other staff and to the general public.

6. Must have CPR/First Aid Certificate.

7. A working knowledge of conducting individual and group rehabilitation sessions for those with a history of mental illness and their families.

8. Know and comply with Agency policies and procedures, HIPAA, Department of Mental Health policies and documentation guidelines, and other state federal regulations relating to service programs for adults with a history of mental illness.

9. Basic advocacy skills.

10. Present ideas, information, and viewpoints clearly, both verbally and in writing.

11. Efficiently use the personal computer to include a variety of word processing software.

12. Utilize analysis, experience, and judgment to make decisions.

13. Demonstrate commitment to team objectives and Fleming and Barnes dba Dimondale Adolescent Care Facility philosophies.

14. Adapt to changing needs by acquiring new skills and knowledge.

15. Current California drivers; license and a driving record acceptable to the Agency’s insurance carrier.

16. Provides services within a culturally sensitive, ethic informed model of practice and is respectful to the community and clients.

These requirements are typically met by a combination of education at a bachelor’s level and experience that includes working with individuals with a history of mental illness in a social service/ mental health environment.
