Job Summary:

CareLinx by Sharecare and a large health plan are partnering to provide thousands of members with in-home care benefits and are looking for a caregiver in the state of Washington. As an Appointment Escorts you will be supporting elderly care by helping with their appointment visit to their doctors. You will play a vital role in ensuring that they complete their check-up, serve as a primary contact person during the visit, assisting them in going to restroom, and assistance with transfer. Rate: $19/hr. for CGs and $22/hr. for CNA

Essential Job Functions:

• Arrive/check in at the doctor’s office 30 minutes prior to the scheduled appointment.
• Stay with the participant from the time they arrive until the time they board their ride home except for the time participant is meeting with the provider-escorts leave the exam room during the examination after assisting with any necessary clothing change and transfers to exam chair/table.

• Provide a packet of information when the participant checks in for their appointment.
• Remind providers to communicate any information about the appointment directly to ElderPlace via phone (206-320-5325) or fax (206-320-5326).
• Provide assistance with transfers.
• Provide assistance with using the restroom, including assistance with urine &/or stool collection as needed.
• Carry a cell phone in case emergencies occur and the escort needs to talk to ElderPlace right away.
• Inform ElderPlace if the participant goes home with a family member, including the family member’s name.
• Call ElderPlace transportation to arrange return ride

Qualifications:

• Previous companionship and personal care experience
• Passionate about child & elderly care with a genuine desire to help others
• Reliable & professional
• Fully vaccinated for COVID-19 with one booster
• Trained in First Aid and CPR
• Trained in Emergency protocols and procedure
• Trained in Dementia, Fire, Safety, and Infection Control
• Minimum of 6 to 1 year experience
