The Youth and Family Specialist for Youth Services provides outreach, engagement, behavioral support and transition services at clinic sites, as well as outreach locations throughout Yolo County, to include home visits, work in schools, and the juvenile detention facility. We are a Federally Qualified Health Center (FQHC) and have been providing care to Yolo County residents since 1972. Like other community health centers, we provide care to anyone that walks through our doors and are located in all major cities within Yolo County (Davis, Woodland and West Sacramento) and provide services throughout Yolo County. The community health center movement was born out of the civil rights movement in the 1960s with the mission to provide healthcare in communities where there are few or no options for care. Our mission is rooted in the philosophy that health care is a right, not a privilege. We serve and provide care for those disproportionally impacted by social determinants of health.

SALARY:

  • The pay range for this position is $62,400-$64,480 annually. Starting pay within this range will vary depending on job-related knowledge, skills, abilities and experience.
  • May be eligible for annual 2% + 0-2% merit-based pay adjustment (0-4%).

OVERVIEW:
The Youth and Family Specialist for Youth Services provides outreach, engagement, skill-building/rehabilitative support, facilitation for Child and Family Team meetings, transitions for continuity of care, and step-down services under Specialty Mental Health. The CommuniCare Health Center's mission emphasizes supportive, nonjudgmental care, preventive care, health education and community referrals to address the broad spectrum of health and psycho-social concerns of clients served. CommuniCare Health Centers is a Federally Qualified Health Center providing comprehensive patient-centered primary care services including medical, dental, substance abuse treatment, behavioral health, and outreach services to low income, multi-ethnic, uninsured and underinsured populations.


SITE/HOURS
: Full-time position, 40 hr/week which may include evening hours; CommuniCare Clinic sites and outreach locations throughout Yolo County (Woodland, West Sacramento, and Davis), to include home visits, work in schools, and juvenile detention facility.


RESPONSIBILITIES: The following reflects managements definition of essential functions for this job but does not restrict the tasks that may be assigned. Management may assign or reassign duties and responsibilities to this job at any time with reasonable accommodation.

  • Conducts screening and intakes for clients and families referred to Youth Services.
  • Provides skill based and rehabilitative behavioral interventions to clients throughout Yolo County in clinic, in the community, in home and in schools.
  • Participates in the family-centered, strength based, culturally relevant, needs driven planning process for youth and families referred to Youth Services.
  • Coordinates and facilitates Child and Family Team meetings with the client, family, natural supports, and formal supports/professionals.
  • Works with the school staff, youth, and their family to facilitate safety planning and individualized youth and family plan.
  • Utilizes evidence-based assessment and outcome measurements tools, as well as compiles and reports data.
  • Completes all client information and documentation in a timely manner (48 hours) according to guidelines.
  • Works with clients to establish long and short-term goals. Specialist is responsible for detecting emotional, psychological, and physical danger signals. Specialist is non-judgmental and offers feedback and support to clients.
  • Responsible for case management, crisis intervention - including de-escalation - safety planning and collateral services for youth and parents/caregivers.
  • Provides direct services to clients and families, providing skills and support to achieve individualized youth and family goals.
  • Maintains a 65%+ level of productivity of direct client service.
  • Responsible for remaining within the ethical standards established in the field, including those of confidentiality.
  • Works as a part of a multi-disciplinary team including Probation, Juvenile Detention Facility staff, CWS, behavioral health clinicians, psychiatrist, youth and parent partners, and other county agencies to coordinate care for youth and families as directed by supervisor.
  • Attends ongoing supervision and weekly staff meetings. Attend training/conferences as arranged.
  • Responsible for maintaining the standards as listed in the CommuniCare Health Centers Personnel Policies and Procedures Manual, and any other standards which may apply to this position.
  • Other projects as may be assigned.

Education: BA required. A minimum of 1 year experience working in a mental health setting required.

Skills & Abilities: Bilingual Spanish/English preferred. Must provide proof of current drivers license and auto insurance.

Expectations: Attendance and punctuality is necessary. Ability to work well with a multidisciplinary team. Ability to problem solve and be self-motivated. Understanding and use of DSM 5. Computer proficiency. Must have a commitment to excellence and high standards. Must have effective and flexible written and oral skills, strong organizational, problem-solving, and analytical skills. Must have ability to manage priorities and workflow. Required to have versatility, flexibility, and a willingness to work within constantly changing priorities with enthusiasm and have acute attention to detail. Expected to have strong interpersonal skills and have the ability to understand and follow written/verbal instructions. Able to work independently with little or no supervision. Required to have the ability to deal effectively with a diversity of individuals at all organizational levels and with external customers.

Physical Requirements:

Ability to operate office equipment, including computer equipment. Must be able to sit or stand for long periods of time, with occasional walking. Requires a moderate level of mobility, dexterity, and flexibility in order to perform essential functions. The employee is occasionally required to stand, walk, bend, reach with hands and arms, and to lift 15 lbs. Hearing and close vision abilities required. Work is performed in a typical office environment with moderate equipment noise. Travel to other agency sites required.Bottom of Form
