For a youth and family coach who comes at difficult problems in new ways, Nexus-FACTS has a unique opportunity to be part of a new, one-of-a-kind treatment team. Nexus-FACTS seeks a Youth and Family Skills Coach for its new, grant funded Mobile Transition Team (MTT). The MTT is a new opportunity to address the needs of youth with high levels of mental health and behavioral challenges who have completed their treatment in a Psychiatric Residential Treatment Facility (PRTF) or the Child and Adolescent Behavioral Health hospital (CABHH) when a discharge placement has not been located. As part of a multidisciplinary team, the Youth and Family Skills Coach will provide skills services in the treatment of children, adults, and/or families as prescribed in the client’s developed treatment plan as directed and supported by the Clinical Supervisor.

Primary responsibilities:

Under the direction of the licensed clinical supervisor, the Mental Health Practitioner I will participate in program planning, implementation, and delivery of skills services for assigned cases which may include:

  • Assist clients in using age-appropriate self-management skills, including communication, self-regulation, self-help skills, play, social, and relationship skills.
  • Provide de-escalation and therapeutic supports as needed with clients
  • Immediate notification and collaboration with Clinical Supervisor in case of crises and emergencies.
  • Completion of clinical documentation in compliance with program expectations; demonstrate strong written and oral communication skills to generate and support program documentation expectations and standards.
  • Attendance and participation in clinical supervision and team case consultation meetings in accordance to program expectations.
  • Completion of education and training experiences in accordance to program expectations.
  • Ensure minimum hours of direct services as assigned.
  • Maintain compliance with all agency, state, and regulatory standards and remain compliant with all HIPAA regulations.
  • Assist in implementing responses to youth behavior and other skills deficits according to the developed treatment plan.
  • All other tasks and duties as assigned.

Required Education and Licensure:

  • Bachelor’s Degree in a degree in behavioral science field, Social Work, or Psychology
  • Must have completed 2,000 hours of supervised clinical experience delivering clinical services to treat mental health and emotional disturbances with a trauma focus
  • In lieu of a Bachelor's degree and 2,000 hours of supervised clinical experience, would consider minimum completion of 4000 supervision hours of supervised clinical experience delivering clinical services to treat mental health and emotion disturbances with a trauma focus
  • 1-2 years’ experience working with at-risk youth and families
  • Ability to pass all necessary background screening requirements per DHS rules and regulations
  • Proficiency in Microsoft Office Suite and data entry skills required
  • Educated on and compliant with HIPAA regulations
  • Valid driver’s license required. Must meet state regulating agency and Nexus Home Office driving requirements
  • Must have access to use of personal vehicle for business travel

The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job, the employee is regularly required to talk and hear. Specific vision abilities required by this job include close vision and ability to adjust focus, and maybe required to lift-up to 50 pounds unassisted and up to 100+ pounds with assistance. Standing and walking for extendedperiods of time (2-4 hours) is required. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.

Expected Hours of Work:

Days and hours of work are Monday through Friday, with the expectation to work specific schedules and hours asidentified and communicated by management. Must have flexibility for evening and some weekend hours.
