We are currently hiring a Wellness Navigator based in Worcester MA. This position will work with refugee patients and communities providing regular Wellness and Integration visits (virtual and or over the phone) to check-in with new arrivals, review engagement in care, isolation, or distress and provide support to engage/re-engage in care. This role will work closely with the healthcare team as informed advocate or liaisons to guide patients through the health care system and connects them to community resources available. The Wellness Navigator is part of the Community Health Worker team.

Job
Summary:

  • Utilizing standard screening tool, grounded in awareness of Social Determinants of Health best practices to create encounters/ visits.
  • Conducts monthly visits to refugees for up to 12 months, at minimum one virtual/telephone encounters for everyone who is referred for RHAP, and up to a maximum of 12 virtual/telephone encounters. Encounters may be initiated prior to the first Refugee Health Assessment visit.
  • Document all encounters in our Electronic Health Record.
  • Request follow up RHS-15 or BHI appointments, in collaboration with the Behavioral Health staff in charge of conducting RHS-15.
  • Track numerous data points, including but not limited to the number of contacts/successful visit, number of patents connected to services such as: primary care, mental health services, food/housing security resources, etc.
  • Assists patients in accessing and utilizing healthcare services, community wellness programs, community resources unknown or otherwise unavailable to members without appropriate referrals that are culturally appropriate.

Job Requirements:

  • Minimum 1 year of work or volunteer experience in human services or healthcare with demonstrated knowledge of or ability to work with the targeted community.
  • Demonstrated experience and/or interest in working with refugee populations.
  • Basic knowledge of Microsoft Office and ability to learn the health center’s Electronic Medical Record and other applications.
  • Excellent written and oral communication skills.
  • Depending on the nature of the position, bilingual in English and another critical language for the health center may be required.
  • Successful completion of the CHW core competency training within 2 years of employment.
  • Bilingual in English and Pashto strongly encouraged to apply.
