Responsibilities

Job Summary:

This position involves the coordination of clinical management of the heart valve patient. Manages and coordinates patient evaluation and education for Structural heart procedures (TAVR, LAAO and TEER). Serves as a leader for the hospital valve program and drives performance and improvement initiatives. Acts as a clinical resource and a role model for all areas caring for structural heart patients. Provides and/or coordinates educational activities for both staff and patients. Acts as a community liaison on behalf of the program and the hospital to promote awareness and education on structural heart. Collaborates in the development and delivery of care for Dignity Health.

Job Responsibilities:

  • Ensures up to date knowledge of practice standards, protocols, procedures and most recent research applicable to the heart valve patient. Maintains and demonstrates clinical expertise in the area of structural heart. Attends training and updates courses and conferences, as appropriate or requested.
  • In collaboration with the program director, develops and implements standards of care and practice pursuant to regulatory guidelines and requirements for the structural heart program.
  • Conducts rounding on patients and communicates with multidisciplinary team members regarding aspects of standards of care and application of clinical practice guidelines. Demonstrates, via role modeling and consultation, advanced knowledge and practice of the heart valve patient.
  • Works with and serves as a resource to staff, as appropriate.
  • Participates in hospital wide committees and task forces as required for program management.
  • Collaboratively works with department director, physicians and interdisciplinary team to develop, implement and revise care pathways, treatment algorithms, order sets and valve clinic processes.
  • Leads program education and promotion through the entire facility.
  • Develops and maintains the operating framework of the heart valve clinic. Develops, creates and revises forms, reports, policies and procedures for the clinic; including patient referral packets, patient education materials and procedure candidacy files.
  • Works with marketing department and business development to promote awareness and education to the medical
    community and the community at large. Participate in outreach efforts, health fairs, etc..

Qualifications

Minimum:

Experience:
3-5 years of clinical nursing experience, preferably in Cardiology, Critical Care, Cardiac Surgery or Emergency Department.
1 year experience in ACC Cardiac Registry, preferred

Education:
BSN

Licensure:
Current AZ RN license
ACLS & BLS,must be AHA.

Special Skils:

Must possess leadership and communication skills to coordinate an interdisciplinary team
Public speaking experience desirable
