We serve all WI tribes by providing administration of grant-funded programs either directly or with sub grants. Tribal LTC Manager


This position works with the Program Director to identify gaps in tribal long term care service delivery networks and works to improve Tribal Elder access to services using a care management model. Duties include development and implementation of care coordination model, management of care coordinators and cultural coordinators, being a liaison between managed care partners, writing of policies and procedures to support care management, and fostering relationships with all tribal stakeholders. Position works closely with stakeholders to assist the Director in strategic processes for successful model development. Regular travel is required to meet the needs of the position.

Principal Duties and Responsibilities:

  • Research tribal Elder long term care practices with partner tribes.
  • Identify quality indicators that support elder independence encouraging social interaction and independent living with caregiver supports.
  • Develop strong working relationships with Elders, health care providers, caregivers, and other stakeholders in tribal communities.
  • Gather data and produce reports as needed.
  • Work with other team members in elder programs to develop and maintain client records, client-tracking records, and client resource directory and database.
  • Maintain knowledge and familiarity of tribal resources, local area resources, and statewide resources.
  • Develop and maintain culturally sensitive informational materials.
  • Contribute to ongoing strategic planning, reporting and program support to ensure the successful development and implementation of the ICCM program.
  • Assist in care coordination planning, assessment, and care plan implementation as part of tribal care management model.
  • Consult with Department of Human Services (DHS), local ADRCs, Tribal ADRCs and tribal health care communities as needed.
  • Actively participate in task teams, working groups, and other joint program initiatives as directed.
  • Prepare, review and forward reports according to the Director and program requirements.

Additional Duties and Responsibilities:

  • Develop a care management process including assessment, care plan development and implementation.
  • Use continuous improvement techniques to improve the quality and efficiency of customer service.
  • Assure all services are delivered in a culturally sensitive manner.
  • Participate on committees outside and within the organization.
  • Attend all training sessions and meetings as scheduled.
  • Attend conferences and trainings to stay current on advances in the field and obtain new skills.
  • Participate in research projects as designated.
  • Perform other duties as necessary to fulfill the responsibilities of this position.

This is a fulltime exempt position working Monday through Friday, 8:00am-4:30pm. This position is located in Lac du Flambeau, Wisconsin. This is not a remote position. This position, based on skills and experience, has a salary of $58,000 annually with benefits including Annual Leave, Personal Leave and Holiday Pay;Health, Dental, Life & AD&D, STD and LTD Insurance, FSAand401(k) Retirement Plan. This position requires frequent travel for the purposes of attending community and program related activities. Drug and background screening is required before hire.


Qualifications:

Required: A bachelor’s degree and/or 4 years of progressive experience in Long Term Care and tribal elder health initiatives including medical, social, and behavioral services. Good understanding of LTC industry and market conditions with very good written, verbal, and organizational skills. Supervisory experience minimum 3 years. Preferred: A degree in the nursing or healthcare field. Experience and knowledge with tribal Long Term Care service delivery. Skills and experience in facilitating small group learning and discussion programs. Experience developing collaborative relationships, preferably with tribal community organizations and community stakeholders. Understanding of the traditional American Indian health issues. Experience working with tribal elders and/or a thorough understanding of elders’ roles in American Indian communities. Experience working with American Indian Tribal Governments, Tribal organization and/or not-for-profit agency. Ability to work independently and as a team member. Individual must possess: Excellent verbal and written communication skills; professional demeanor and appearance; problem solving skills; ability to manage time effectively; self-motivation skills; work effectively under pressure; ability to build positive work relationships with internal and external customers; ability to interact professionally with a wide variety of people; ability to speak in front of groups. Individual must be able to address a wide range of intellectual and practical problems, apply statistical calculations to findings, and the ability to compose complex business correspondence (reports, health profiles). Requires a high level of confidentiality. Must possess valid driver’s license and/or reliable transportation with adequate insurance and the ability to travel daily.
