Scope: The Program Manager is responsible for well-planned and coordinated recovery-based services within the case management community program. The Program Manager provides supervision to the Independence Coaches and overall management of the Transitional Housing program. The Program Manager will ensure that each client within the assigned caseload is sufficiently supported by the Skyland Trail program services from the client’s entry into the Independence Coaching program until discharge.


Duties

Functions as an Independence Coach for assigned caseload and coordinates all aspects of services to support the goals of these clients (i.e. recovery plan meetings, treatment planning, and discharge planning).


Develops individualized recovery plans for caseload and revises as necessary with input of recovery team. Conducts individual case management sessions at designated frequency determined by level of care with assigned caseload, also reviews clients’ treatment plans and documents progression towards recovery plan goals. Records all changes in status as necessary.


Leads weekly recovery team meetings and makes professional case presentations. Communicates recovery goals, wellness plans, expectations and opportunities for clients.


Supervises Independence Coaching and Transitional Assistant staff. Provides administrative and clinical support for direct report(s), completes annual evaluations, implements progressive disciplinary action, if needed. Completes payroll for Independence Coaches and Transitional Assistants.


Participates in the Safety Committee and completes monthly inspections with the Support Services Manager.


As a part of the Independence Coaching program, oversees the activities and programming at North Transitional Housing and meets weekly with the Transitional Housing Recreational Assistant.


Along with the West Day Services Director, assigns clients to appropriate Independence Coach caseloads as needed.


Participates in weekly supervision with Medical Director. Participates in weekly supervision with West Day Services Director for administrative purposes, support and case consultation.


Works with recovery team to design wellness plans. Set appropriate wellness plans for caseload as needed, and reports plan to the client, family and clinical staff.


Completes weekly budget calculations and maintains receipts for reimbursement for money used in Transitional programming. Turns in a report of monthly expenses to the finance department.


Maintains verbal or written contact with families of assigned caseload at least once a month.


Practices ethically and complies with facility policies and accreditation standards (i.e., Joint Commission, etc.)


Assists and guides clients to achieve the maximum level of independence, while functioning in the least restrictive environment.


Refers clients to appropriate services, internally and externally. Maintains contact with other service providers to introduce the program and follows up in times of crisis.


Conducts assigned groups and documents in electronic health record. Develop written curriculums for each group. Utilize curriculum resources to plan, facilitate and/or co-facilitate assigned groups and/or outings.


Participates/facilitates a weekly community meeting with residents.


Utilize a schedule for administering drug tests to clients in the program consistently.


Keeps up to date documentation of upcoming moves/transfers in and out of the program.


Works with the facilities team to complete monthly walk through, safety checks, and fire/disaster drills.


Provides transportation for clients as needed.


Participate in continuing education in supervision, case management, etc.


Completes other duties as assigned.

Qualifications:

  • Master degree in related field (i.e.; Psychology, Social Work, Counseling, etc.).
  • Prior experience in partial hospitalization/day treatment setting for chronically mentally ill.
  • Case management and/or management experience preferred.
  • License preferred.
