OVERVIEW OF THE POSITION

Under the direction of the DRC Program Manager, the Transitional Housing Monitor provides case management services to DRC program participants residing in Transitional Housing. The Transitional Housing Monitor provides day to day case management activities of the Transitional Housing population (including adherence to programming) and assists in addressing linkages to other necessary systems of care (e.g., primary medical care, child care, educational and vocational needs, and permanent housing

RESPONSIBILITIES/DUTIES

The following responsibilities and duties are general in nature and are intended to define a framework of job responsibilities and duties for the employee. All Center Point employees are responsible to carry out assignments in a professional manner including thoroughness and timeliness.

PRIMARY DUTIES

  • Provides direct case management services including assistance in meeting and maintaining the requirements of the Transitional Housing provider.
  • Provides assistance with establishing eligibility and making application for public benefits. 02 Conducts housing eligibility screening and orientation prior to referring to Transitional Housing.
  • Makes referrals for supportive services for housing participants in accordance with service plan.
  • Monitors activities in transitional housing facilities to ensure that DRC clients are adhering to the rules and structure of the housing program and that the housing facilities meet all required standards [cleanliness; structure; meals or food service; etc.].
  • Conducts random UA testing on DRC participants residing in Transitional Housing programs.
  • Conducts client outreach and engagement activities.
  • Conducts independent living skills workshops.
  • Provides information and referrals for permanent housing.
  • Maintains documentation of DRC client participation in housing services and progress toward securing permanent housing.
  • Provides advocacy to assist the participants in obtaining appropriate support services and permanent housing.
  • Refers participants to appropriate community-based services as necessary (i.e., housing, vocational and educational services, primary medical care).
  • Coordinate with participant and community based treatment providers, and other service providers to ensure participant needs are met.
  • Interact with staff and participants in a culturally sensitive manner.
  • Conducts follow-up and other client tracking activities.
  • Assists in maintaining a safe and secure environment within the DRC.

Job Type: Full-time

Pay: From $24.00 per hour
