Incumbent ensures continuity of services to citizens of the Middle Peninsula Northern Neck who have an intellectual and/or developmental disability. Support Coordinators assist individuals with coordinating resources to address their basic physical, financial, social and personal needs. Under general supervision, employee provides Person Centered Services including the development of a person centered plan, service provider linkage, monitoring, advocacy, and other supports to encourage and enhance community integration. The employee provides these services to these individuals following their Person Centered Plan. When appropriate, work involves coordinating the development of individual behavior management and skill training programs. Employee is responsible for monitoring the individual progress of assigned caseload and making modifications to the plans as necessary; performing individual assessments to determine needs; monitoring residential, day support and other service records and maintaining Person Centered Plans and case management records in compliance with standards and policies. Employee makes referrals as appropriate and links with community resources. When needed and appropriate, employee arranges for transportation to and from needed locations. Employee must also exercise tact and courtesy in contact with staff, individuals, families, and other service providers and community resources.

Bachelor's Degree in social work, case management, intellectual disabilities health assessment, or related field, with preferably experience working with individuals with intellectual disabilities.
