ABOUT THE ROLE

We’re currently looking for a Substance Use Disorder Care Manager for our integrated primary care office, the Pittsburgh Mercy Family Health Center to serve on our care management team. Our care managers have in-depth knowledge and experience in the care coordination of individuals experiencing serious and persistent mental illness, substance use disorder and/or housing instability.

The Substance Use Disorder Care Manager coordinates the scheduling and eligibility requirement for individuals enrolled in the Pittsburgh Mercy Family Health Center Medication Assisted Treatment Program while being the point person for all office based substance use disorder and forensic concerns. Our care managers develop trusting relationships with persons served through high-touch engagement in the primary care office and utilize patient registries to track our high risk patients.

RESPONSIBILTIES/REQUIREMENTS:

  • Master’s Degree in Human Services and 1 year of community-based health services deliver or a bachelor’s degree in Human Services and 3 years of community-based behavioral health service delivery
  • Work Schedule: 37.5 hours per week, typically Mon to Fri 8am – 4pm, with one evening per week
