Signal is seeking an Involuntary Commitment (IC) Care Coordinator who will be responsible for the support and coordination of care for those that are involuntarily committed to treatment for a substance use disorder. This role will support Signal’s commitment to ensuring the availability and access to high-quality behavioral health services in the regionally contracted counties on behalf of the State of Colorado’s Behavioral Health Administration (BHA). The IC Care Coordinator will serve as a vital member of the Signal team and will help in agency and individual monitoring, navigating and linking individuals to clinically appropriate services and resources.

Specific Duties:

  • Cross-department coordination related to care delivery and quality monitoring.
  • Working collaboratively with the BHA to perform the coordination of care with individuals who have been placed on an IC. Coordination can include, but is not limited to, securing placement at a withdrawal management facility, admitting into treatment, accessing ancillary services, connection to recovery services and supports, and transition of care coordination. This will be achieved mainly at a system and provider level.
  • Document all care coordination activities in the electronic health record.
  • This position is predominantly remote but may require the occasional in-person meeting.
  • Attend, at minimum, virtual monthly case conferences with providers and BHA.
  • Support the collection of required provider reports.
  • Provide presentations, training and technical assistance to providers and community organizations as needed.

Education and Experience:

  • Bachelor’s degree in a relevant area such as Social Work, Counseling, Marriage and Family Therapy, or another related field and minimum of two years of experience in assessment, treatment, and case management of those with a substance use disorder. Master’s degree preferred.
  • CAS or LAC required.
  • Knowledge of the mental health and substance use delivery systems including State and/or local government programs, behavioral healthcare entities, and related organizations.

Required Skills:

  • Experience in behavioral healthcare service delivery (with an emphasis in substance use disorders) and coordination of care across systems.
  • Practical and theoretical application of ASAM.
  • Ability to prioritize, work independently, and research solutions with minimal direction.
  • Ability to transport self to work and to various work assignments throughout Colorado.
  • Ability to pass background check.
  • Valid driver’s license.
  • Strong, effective, and responsive written and verbal communication skills.
  • Excellent customer relations skills.
  • Good judgment, tact, initiative, and diplomacy to make effective independent decisions.
  • Organized, attention to details, and ability to manage a variety of tasks at the same time.
  • Proficiency in Microsoft Excel, Outlook, Word and PowerPoint, and ability to learn other electronic systems.

Job Type: Full-time

Pay: $52,000.00 - $60,000.00 per year
