What Will you be Tasked With?
  • Demonstrates knowledge of the principles and practices of speech pathology and provides speech therapy services in accordance with standards of care for speech language pathology and the Health Care Provider’s orders.
  • Performs an initial assessment and evaluation of patient’s speech and language and reports evaluation findings to the Health Care Provider and clinical manager promptly, and assists in establishing a plan of care.
  • Plan for coordinated service for patients by consulting and working with other health care team members effectively and communicating schedules as assigned.
  • Observes, records and reports to Health Care Provider the patient’s treatment, response and progress and promptly reports significant changes and obtains orders to revise plan of care, and documents accordingly.
  • Documents initial evaluation and treatment plan and re-evaluations of patient’s progress at least every thirty (30) days and updates plan of care as necessary. Completes comprehensive assessments and OASES data, as indicated on a timely basis.
  • Counsels and teaches patients and their caregivers regarding patients’ speech and language disturbances and provides support and educational materials to promote progress.
  • Demonstrates activities and exercises to the patient, caregiver or nurse who may assist the patient and provides written instructions as indicated to promote effective compliance.
  • Informs the clinical manager of any unusual occurrences, accidents, or adverse effects of medical devices and documents accordingly.
  • Completes clear concise and accurate clinical notes, which document skilled services appropriately, and submits all requested documentations on a timely basis (24hr).
  • Assists in referring patients to speech clinics and other community agencies as needed.
  • May supervise CHHA in procedures directly related to the speech-language therapy plan of care.
  • Recommends and assists patients to obtain needed DME, communication devices, safety modifications, etc. and refers to social services as indicated in accordance with the Health Care Provider's plan of care.
  • Participates in orientation, case conferences, in-services and staff meetings as requested.
  • Provides individual and group staff education programs, to prepare nurses and other disciplines to identify and assist patients with their speech, language, swallowing, and communication problems, as necessary.
  • Uses proper infection prevention and control procedures, universal precautions and safety measures.
  • Serves as a member of the Professional Advisory Committee and participates in quality performance/improvement activities as assigned.
  • Adheres to agency’s policies and procedures.
  • Provides and documents services in accordance with applicable State, Federal, Medicare, Medi-cal regulations and standards and third party payer guidelines.

Environmental Conditions and Physical Requirements
  • Driving or riding in a motor vehicle, standing, sitting, walking, bending, reaching, and stretching.

Modesto/Merced/Turlock/Los Banos

Minimum Requirements:
    • Master's degree in Speech Language Pathology
    • Active Speech Language Pathology license (CCCs)
    • Ability to practice speech therapy unencumbered within state and federal guidelines
    • At least one year of experience working as a Speech Language Pathologist (no CFYs)
    • Must have vehicle, current driver’s license and appropriate automobile insurance.
    • Ability to make home visits and provide speech therapy evaluations and treatments in the home environment complete documentation effectively and submit in a timely manner.
Preferred Requirements:
    • Home health experience
    • Home Care Home Base documentation experience
