What is a Specialty Case Manager?

A Specialty Case Manager at Community Mental Health is responsible for oversight of court ordered mental health treatment and Assisted Outpatient Treatment (AOT) process for individuals who are non-compliant and/or not established with the agency.

They complete community referrals as needed and coordinate care with established treatment providers, ensure legal documentation is thoroughly completed and submitted to Probate Courts within a timely manner, and provide training and act as liaison for agency jail diversion programs.

What shifts are available? Monday-Friday, 8am-5pm with occasional rotational on-call services.

LOCATION:

  • This posting takes place on-site. Due to the nature of the work, remote employment is currently unavailable for this position.

QUALIFICATIONS

Education:

  • Nursing degree or bachelor's degree in Social Work from a recognized or accredited institution. Must possess state license or registration as required.

Experience:

  • Lived experience with mental illness, intellectual/developmental disabilities, and/or substance use disorders valued.
  • Experience in working with Probate Courts and courted ordered mental health treatment preferred.

Other:

  • Must be at least 18 years old.
  • Must possess a valid driver’s license and acceptable driving record (as determined by the agency).
  • Strong communication skills and computer literacy required.

RESPONSIBILITIES:

As a Specialty Case Manager you will:

  • Be responsible for oversight of court ordered mental health treatment per deferral and Assisted Outpatient Treatment (AOT) process for individuals who are non-compliant and/or not established with the agency.
  • Attend and participate in inpatient treatment team meetings to assist with facilitating deferral and AOT planning process.
  • Assist individuals in accessing and maintaining a continuum of mental health services as outlined in court ordered treatment plan in order to ensure treatment compliance and remain as independent as possible.
  • Complete core functions including assessment, service plan development, linking, coordination of services, follow-up, and monitoring of services on a short-term basis.
  • Work closely and collaborate with agency court liaison, hospital discharge planner, psychiatrists, and other agencies as required.
  • Be responsible for filing legal documentation in a timely manner and notifying Probate Courts of non-compliance status.
  • Demonstrate accurate assessment and understanding of diagnoses, utilizing the Diagnostic and Statistical Manual of the American Psychiatric Association.
  • Demonstrate effective case recording in accordance with Person Centered Planning and NeMCMHA "Case Recording Guidelines".
  • Demonstrate knowledge and understanding of legal documentation and Michigan Mental Health Procedures, i.e. Blue Book.
  • Develop and maintain professional relationships with local probate courts and assigned legal representation.
  • Demonstrate knowledge of the legal, regulatory, and administrative requirements governing the provision of court ordered mental health treatment according to the Michigan Mental Health Code.
  • Conduct regular direct and in-direct follow up meetings to assess needs and treatment compliance of assigned caseload.
  • Complete community referrals as needed and coordinate care with established treatment providers.
  • Ensure legal documentation is thoroughly completed and submitted to Probate Courts within a timely manner.
  • Provide training and acts as liaison for agency jail diversion programs.
  • Monitor court ordered treatment for individuals not established with NeMCMHA.
  • Participate in the agency's emergency services unit (ESU) on-call rotation.
  • Be an active member of the Children's Diagnostic and Treatment Team.
  • Demonstrate knowledge of universal precautions, standards of practice, and emergency protocol.
