JOB REQUIREMENTS

Minimum Education: Bachelor degree in Social Work from an accredited program (C.S.W.E.)

Minimum Work Experience: Previous experience with hospital social work is preferred

Required Licenses / Certifications: BLS, Current license as Licensed Social Worker in the State of Idaho

Required Skills, Knowledge, and Abilities: Able to work independently with minimal supervision, ability to be flexible and prioritize tasks Able to exhibit controlled emotional involvement in highly stressful, emotionally laden situations. Strong interpersonal skills are required with ability to communicate effectively both verbally and in writing.

BASIC PURPOSE OF THE JOB

The Medical Social Worker- LSW is responsible for providing comprehensive social work and care management to patients, families by utilizing holistic assessment and appropriate intervention strategies that promote improvement of psychosocial problems, efficient use of medical resources, and facilitate safe and appropriate discharge planning and care management as it relates to specific medical disorders and psychosocial needs of patient and their family. The medical social worker provides social work services in hospital during after hours, weekends and holidays as part of the department on-call schedule. The social worker is under the supervision of the Director of Social Services- Utilization Review.

ESSENTIAL FUNCTIONS

Recognizes and attends to psychosocial needs of patients and families. Maintains updated knowledge of community resources.
Maintains timely and accurate social work documentation. Continues development of expertise in profession. Active participation in process and performance improvement. Acts as patient advocate regarding plan of care and potential discharge disposition/care needs. Works well with staff members.
