General Statement of Job

Under general supervision, serves as lead worker in performing social work and case management in the provision of social services for individuals in need of assistance. Employee is responsible for investigating allegations of abuse or exploitation of elderly/disabled adults, investigating cases of suspected child abuse/neglect, assisting clients in achieving self-sufficiency, or implementing activities associated with other social services. Work involves providing direction to other Family Services Worker, assigning cases to co-workers, performing case management, providing crisis intervention and/or counseling, assessing client needs, coordinating provision of needed services, and maintaining case records.

This position is essential. Personnel occupying this position may be scheduled to work irrespective of weather conditions and/or when general county offices are closed.

Special Duties & Responsibilities

  • Provides direction, training, and support to other Family Services Worker; coordinates assignment of cases and daily work activities; conducts unit meetings and case staffings; monitors status of work, inspects completed work, and troubleshoots problem situations.
  • The Primary role of this position will be responsible for completing adoption home studies; maintaining adoption subsidy records for each subsidy case; updating payment records for all subsidized adoptions;
  • Performs case management, intervention, and/or counseling involving situations such as abuse, neglect, or exploitation of children, elderly citizens, disabled/incapacitated adults, or other individuals at risk; implements activities associated with other social service programs, such as assisting clients in achieving self-sufficiency.
  • Investigates allegations of abuse or exploitation of elderly/disabled adults; conducts assessments of personal care facilities, nursing homes, adult care residences, and adult day care facilities; assesses client needs and makes provisions for services, which may include protective services, companion services, or long term services; recruits, interviews, and approves companion aides, and monitors companion aide cases; makes home visits to clients' homes; reviews guardianship reports.
  • Investigates cases of suspected child abuse/neglect and makes provisions for protective services; conducts court-ordered custody investigations and home studies, and monitors court-referred families; provides court-ordered mediation services for custody investigation disputes; compiles records, gathers information, and provides oral/written testimony to courts; responds to courts regarding child-related questions/issues; evaluates ability of parents to adequate care for children; removes children who are victims of abuse/neglect from homes or as ordered by court.
  • Works to prevent out-of-home placements (foster care) for children through the court system; visits children/families in home and school environments; conducts family group counseling when child is at risk of out-of-home placement; teaches parenting skills and discipline techniques; encourages families to follow through with services to avoid out-of-home placements; formulates service plans offering appropriate services to children/families; refers families to outside agencies to meet their needs; completes adoption home studies; maintains adoption subsidy records for each subsidy case; updates payment records for all subsidized adoptions; enrolls/dis-enrolls children into/out of school.
  • Coordinates foster placement when necessary; approves foster homes for placement of foster children; assists foster parents in providing for foster children; monitors visitations between foster children and biological families; updates payment records for all foster care cases.
  • Manages caseload of VIEW clients and daycare clients to assist clients in achieving self-sufficiency; assesses needs of VIEW clients and determines appropriate supportive services; meets with clients who have previously been assessed; assigns clients to work activities; interviews daycare applicants; assists clients in dealing with daycare providers and obtaining information; prepares and submits purchase orders and invoices for daycare providers.
  • Represents the agency at various meetings or on various committees/boards; coordinates or participates in meetings relating to area of assignment, such as Individual Education Plan meetings, adult prescription team meetings, Student Assistance Team, or team meetings between family members and other professionals.
  • Prepares referrals for families requiring financial assistance or other assistance; collaborates or contracts with outside agencies or professionals as appropriate; maintains current listings of community resources, social service agencies, or other resources available for clients.
  • Prepares and maintains confidential case records and files; documents all interactions with clients; prepares purchase of services orders and vendor invoices; enters data into computer system.
  • Applies general policy to specific case situations; researches policy as needed.
  • Refers cases to Commonwealth Attorney's Office and law enforcement officials as appropriate; works with police and Commonwealth Attorney's Office regarding joint sexual abuse investigations.
  • Maintains statistical data relating to area of assignment.
  • Assists in coordination of agency special needs funds and special projects.
  • Prepares or completes various forms, reports, correspondence, grant proposals, statistical reports, case records, client activity reports, service plans, uniform assessment instrument, needs assessments, court reports, adoption reports, custody reports, court-ordered home studies, child support reports, purchase of services orders, invoices, provider compliance forms, applications, funding reports, charts, training/educational materials, or other documents.
  • Receives various forms, reports, correspondence, statistical data, case records, purchase of services orders, invoices, applications, court orders, needs assessments, guardianship reports, adult protective services reports, individual education plans, sexual trauma reports, police/arrest reports, medical exemption forms, attendance records, daycare rate sheets, laws, policies, procedures, manuals, directories, reference materials, or other documentation; reviews, completes, processes, forwards or retains as appropriate.
  • Operates a computer to enter, retrieve, review or modify data; verifies accuracy of entered data and makes corrections; utilizes word processing, spreadsheet, database, e-mail, Internet, or other computer programs; performs basic maintenance of computer system and office equipment, such as backing up data or replacing paper, ink, or toner.
  • Communicates with supervisor, employees, other departments, clients, family members, companion aides, social service agencies, government agencies, counselors, medical providers, nursing homes, assisted living facilities, home health agencies, probation officers, law enforcement personnel, court personnel, attorneys, schools, community organizations, professional colleagues, the public, outside agencies, and other individuals as needed to coordinate work activities, review status of work, exchange information, or resolve problems.
  • Provides information and education to the community regarding area of assignment; makes presentations to schools and community organizations.
  • Maintains confidentiality of departmental issues and documentation.
  • Operates a motor vehicle to conduct work activities.
  • Performs general/clerical tasks, which may include answering telephone calls, entering data into computer, typing documents, making copies, sending/receiving faxes, filing documentation, or processing incoming/outgoing mail.
  • Provides assistance to other employees or departments as needed.
  • Assists with outreach and communication to the community; obtain speakers from educational and business fields.
  • Coordinates job/resource fairs.
  • Performs other related duties as required.

Minimum Training & Experience

Bachelor's Degree in Social Services, Sociology, Human Services, Psychology, or closely related field; supplemented by three (3) years of previous experience and/or training that includes social services work and case management; or any equivalent combination of education, training, and experience which provides the requisite knowledge, skills, and abilities for this job.
