SUMMARY:

The Addictions Counselor under the Program Director guidance is responsible for monitoring and delivering high quality services to dually diagnosed/chemically dependent person in recovery (PIR). Primary responsibilities include the provision of a safe and-therapeutic milieu; assessment and progress/digress in treatment; delivering group educational services, crisis intervention and full participation with the multidisciplinary team toward meeting a PIRs treatment plan objective.


ESSENTIAL POSITION REQUIREMENTS:

  • Provides comprehensive ASAM assessments with regards to needs and the appropriate placement into the program; develops individualized treatment plans, which respond to the needs of the person served as identified by the assessment and coordinates the initial and periodic treatment plans with the Medical Services component; determines from assessment the appropriate services to be provided, the most appropriate service providers, and ensures that the person served is linked with those services
  • Maintain client records and completes required documentation according to agency and accreditation standards. This may include, but is not limited to biopsychosocial assessment, IDAS, transition checklist, master treatment plan, individual notes, discharge summaries and recovery plan update, and intake evaluations and documented summaries.
  • Complete Community Behavioral Health (CBH) reviews for continue stay and/or level of care.
  • Ensure all documentation is recorded in electronic health record system.
  • Complete significant incident reports and submit to CBH within 24hrs of learned incident.
  • Presents individual counseling session based on the areas of Relapse Prevention, Addictive Behaviors, and Co-occurring disorders.
  • Completed Individual and Group Counseling.
  • Case Consultations with interdisciplinary team ensuring appropriate aftercare placement and smooth transition to next level of care.
  • Obtain client records from prior treatment experiences, medical records and any other useful written information that may aid ongoing treatment.
  • Assists with Case Management duties, which include one on one client contact each shift, team meetings, collaboration with other service providers and follow up with referral sources.

A Bachelor’s Degree from an accredited college with a major in chemical dependency, psychology, social work, counseling, nursing (with a clinical specialty in the human services) or other related field and 1 year of clinical experience in a health or human service agency, preferably in a drug and alcohol setting.

  • Demonstrate proficiency in Microsoft Word and Excel at the intermediate to advanced level.
  • Excellent time management and organizational skills
  • Advanced oral and written communication skills to effectively problem-solve.
  • Must be a team player, patient, flexible, and can multi-task.
  • Demonstrated ability to complete multiple tasks in a prompt and accurate fashion in a fast-paced environment.
