RN Nursing Supervisor - PMs OR PM/Day shifts

We are seeking an experienced Registered Nurse with proven leadership skills to oversee the care of residents in long-term & rehab settings, primarily on 2nd shift but weekend day shifts are also available. Outstanding assessment and strong communication skills required; this position works closely with MD’s and provides guidance for staff. Duties of the RN Nursing Supervisor will include:

• Assess residents, identify problems and formulate resident goals and nursing intervention.
• Report change in resident condition to attending physician and family.
• Identify emotional and social needs of residents and coordinate plans of care with appropriate department such as Social Services, Chaplaincy or Activities.
• Review medical orders and plans of care with physician and discuss changes based on resident conditions.
• Communicate with families the residents care needs and support residents and family in time of need.
• Assist in training, supervising, counseling and evaluating assigned staff.

Shifts available are 4 - 10:15 pm weekdays, and either 2 - 10:15 pm or 6 am - 2:15 pm weekends. It is also possible to combine the weekend shifts for additional hours.
