Home Helpers Home Care of Leesburg is currently seeking a Registered Nurse for our Client Care Coordinator position. As a Client Care Coordinator, you will assist the administration staff with the supervision of our Home Health Services to a variety of clients.

Schedule: 9am to 5pm Location: Hybrid, You will be splitting your time between working from the office, and site visits with clients.

- Start of care assessments, and onboarding of new clients
- Developing an individualized care plan for each client.
- Client Assessments
- Following up with clients and maintaining a relationship with them.
- Maintain updated Client and Caregiver training information with Administrative Staff
- Attend staff meetings and training sessions as requested
- All other duties as assigned by Administrator and the job description
- Graduate of an approved school of professional Nursing and currently licensed within the state of VA
- Three to five (3-5) years of home health care nursing is preferred and general nursing required
- Good communication, computer, organizational, and time management skills are essential
- Must have prior care management experience
- Ability to travel between office location, and client location.
- Maintain a professional appearance and demeanor
- Strong work ethic and a person who truly enjoys meeting new people and understanding their needs.
