Together Homecare is looking to add a driven Registered Nurse Case Manager to our Greenwood team. This role combines patient assessments with interdisciplinary care coordination, supervision, and care planning, creating a dynamic workflow and supporting a healthy work-life balance. Requirements:

o Registered nurse with an active Indiana license

o 1+ year of nursing experience preferred, but not required. NEW GRADS WELCOME!

o Reliable transportation with an active Driver’s License

Expectations:

Manages all elements of the course of homecare, including care planning, goal evaluation, coordination of care, and care supervision/evaluation.

o Ensures timely and accurate communication and documentation, both in the office and in the field.

o Provides education to external team members, patients, families, and referral sources.

o Participates in care coordination and transition management activities across the healthcare continuum.

o Ensures effective communication to patients, family members, and physicians.

o Participates in staff meetings, required in-services, and agency performance improvement activities as requested.

Due to our great culture, it is preferred that you spend your work week with us in the office, however Together Homecare will provide the flexibility to allow some work from home hours given that you have an appropriate home set up.

o $65,000 - $70,000 starting salary, paid weekly

o Mileage reimbursement at the maximum federal rate
