Pay: $16-$18 an hour.

Schedule: Full-time, Monday-Friday, 11a-7p, required to work one weekend a month, rotating holidays, remote position, work from home.

Referral Hub Assistant Responsibilities:

• Provide timely reviews of clinical and financial elements of the prospective new resident’s case to determine if this patient qualifies for care within the Axiom network.
• Send secured HIPAA information through fax to the appropriate facility under Axiom’s network.
• Log every referral into the referral tracking system.
• Understand the type of resident referrals the facilities under Axiom’s network are willing and able to safely care for.
• Develop strong working relationships with facilities leadership teams and other key employees within the Axiom network.
• Perform other duties as required.

Referral Hub Assistant Qualifications:

• Current LPN (Licensed Practical Nurse), CMA (Certified Medication Aide), or SSD (Social Services Designee) license required.
• 3 or more years of nursing and/or MDS experience in the long-term care environment.
• Must be familiar with the reimbursement process of Medicare/Medicaid and insurance.

Job Type: Full-time

Pay: $16.00 - $18.00 per hour
