POSITION SUMMARY
The Quality Certified Medical Assistant works with the Primary Care Provider Team and the Outreach and Engagement Team to facilitate a comprehensive Telehealth visit. The Quality CMA is expected to review and complete, documentation related to pre-visit planning and care delivery for Transition of Care visits (TCM), Annual Well visits (AWV), Emergency Department (ED) follow-up visits and a Gap in Care Closure visits. Through these interventions the VCMA will ensure optimum patient-to-doctor flow.
KEY RESPONSIBILITIES

  • Reviews and prepares medical record to ensure completeness prior to patient appointment, obtains reports/ results that are not included in the chart. Documents visit, observations, and related activities.
  • Assists with gathering data for the patient history and physical assessment.
  • Checks patient vital signs and body measurements when Bluetooth capability is activated and available.
  • Documents patient stated information: vital signs, pain assessment, height, weight in patient chart.
  • Explains telehealth visit instructions to patient, as appropriate. Provides teaching to patients and families within scope of knowledge base.
  • Assists with scheduling/referral of patients for specific treatments.
  • Participates in interdisciplinary workgroup(s) with Providers, Staff, IT, and outreach staff to co-develop system-wide standards and guidance
  • Identify growth opportunities and performance improvement potential
  • Completes clinical tasks needed to prep patients for Transition of Care (TCM), Annual Well Visits (AWV), and Emergency Department (ED) follow-up visits Telemedicine visits before appointments.
  • Communicate important information to key stakeholders proactively to keep providers abreast of telehealth findings.
  • Track progress and outreach to patients as needed.

Qualifications / Required Skill:

  • COMMUNICATION SKILLS: strong interpersonal, critical written skills required, timely, effective remote communication skills.
  • COMPUTER SKILLS: Excellent computer skills, particularly related to Microsoft applications, including Excel, Word, and Outlook. Comfort with charting, and exploring documentation in Electronic Medical Records and Scheduling Systems.
  • EDUCATION: Associate degree preferred. Certified Medical Assistant required. CPR certified required
  • EXPERIENCE: Minimum Three (3) years’ experience in a Clinical area required. The ideal candidate will have had experience in telemedicine documentation. In addition, the candidate should be able to understand and communicate the connection between telehealth and regulatory requirements.

Education: High School Diploma or Equivalent. Associate degree preferred.
Experience: Minimum Five (3) years’ experience in a Clinical area required.
Licenses or Certifications: Active Certified Medical Assistant required and CPR certified
