Colorado Visiting Nurse Association has an immediate opening for a Physical Therapist. Sign-on bonus $3000.

Location: Designated to one of the following territories, based on where you are located: SW (Lakewood), SE (Aurora), NW (Arvada), NE (Thornton), Central (Denver), CO Springs, Boulder, NOCO (Longmont/Erie)

Status: Full Time, Per Visit - Benefit Eligible

Schedule: Monday-Friday, 8-5

Per Visit Rates: $80,000 - $100,000 estimated annual salary

Sign-on bonus: $3,000

Supplemental Pay: Based on position, schedule and/or availability: Paid Mileage/Shift Diff/Stipends

Mileage and Expense Reimbursement: Maximum Allowable Federal Mileage Reimbursement Rate

REWARDING WORK YOU WILL DO:

Responsible for performing physical therapy, evaluating client needs and conducting case management for patients under the direction of a licensed physician.

  • Provides initial and ongoing assessments to determine level of function, including OASIS assessments at appropriate time point, safety evaluations and recommendations for home modifications.
  • Performs specific therapeutic exercises as needed. Develops an individualized exercise/therapy program based on specific patient diagnosis, goals, and plan of care. Consistently evaluates developed protocols to ensure quality of care. Exercises discretion in determining deviation from methods and protocols to meet health and safety exigencies.
  • The procedures required by the therapist may include (but are not limited to): ultrasound treatment, application of heat and cold, massage, gait training, muscle stimulation, assistance, and training in cardio- pulmonary exercise, mobilization, assistance and training with ambulation, assistance and training with balance, range of motion exercises, patient transfer, strengthening techniques, electrotherapy, prosthetic training, and adaptive equipment.
  • Advises and consults with the family, physician and other agency personnel, including discharge planning.
  • Provides supervision over the Physical Therapy Assistants and Certified Nursing Assistants.

WHAT WE ARE GOING TO LOVE ABOUT YOU:

  • Minimum Education: Graduation from a four-year college or university, including completion of a physical therapy education program.
  • Minimum Experience: At least one year of physical therapy experience required, with two years of SNF, hospital or inpatient rehab preferable. Home health experience also preferred. Other requirements may vary by case or department needs.
  • Required License: Must have active Colorado PT license. Current CPR certification based on individual affiliate requirements.

PHYSICAL REQUIREMENTS:

  • Ability to lift minimum of 50 lbs.
