Foundations Health Solutions has a skilled rehabilitation and long-term care facility located in Galion, OH and is currently seeking an energetic and compassionate Physical Therapist Assistant to provide functional rehabilitation to both our short-term and long-term residents. This position prioritizes quality resident care and overall job performance, and minimizes attention to the traditional productivity measures placed on therapists. The ideal candidate will understand and strive to focus on resident-centered goals that are reflective of a successful return to prior levels of function and best quality of life for our residents.

Responsibilities include, but are not limited to:

· Delivering skilled therapy treatments to short-term and long-term residents that have an established occupational therapy plan of care

· Completing quality and timely documentation, such as daily treatment notes, weekly progress reports, and insurance updates

· Conducting quarterly screens on long-term residents

· Assisting with insurance authorizations and appeals

· Attending all required facility meetings and in-services, including Medicare meetings, patient staffing, family meetings, care plans

· Assisting the facility in meeting the standards necessary to obtain state, federal, and other appropriate certifications

· Presenting themselves in a professional manner at all times including professional conduct, professional communication and appropriate dress while in the facility.

· Maintaining a clean and organized work environment

· Performing other related duties as directed by the Therapy Director, and/or person of authority
