SCOPE OF POSITION: Interacts directly with residents and are an integral part of the resident experience. Personal Care Partners deliver high quality, person-centered care in a fast-paced environment. By developing authentic relationships with residents and family members, helps residents lead lives of enrichment and ensure their wellness needs are met.

ESSENTIAL JOB FUNCTIONS:

  • Assures quality of care by meeting residents needs as outlined in the individual care plan.
  • Communicates and works professionally with the interdisciplinary team.
  • Supports the Wellness Coordinators and the Wellness Manager in performing tasks to assist with resident care in the Ernest P. Barka Assisted Living Community.
  • Supports residents in doing as much for self as possible, promoting positive leisure time.
  • Actively engages residents by initiating conversation and activities with them.
  • Communicates resident well-being and needs by taking an active role in plan of care meetings.
  • Reports concerns of physical, psychological, and social changes to Wellness Coordinator or Wellness Manager.
  • Complies with established nursing standards, policies and procedures of the Assisted Living Department and the facility.
  • Maintains resident confidentiality and privacy.
  • Provides hands-on care to residents as needed and appropriate, assisting with meals and answering nurse call system in a timely manner.
  • Assists residents with nutrition and delivery of meals with appropriate documentation.
  • Provides environmental and personal safety involving adaptive devices and special clothing including application and monitoring and documentation.
  • Demonstrates computer competency as evidenced by accurate and complete documentation using Electronic Medical Record (EMR) system for all daily charting and progress notes on assignment.
  • Maintains a clean working environment by keeping residents living areas neat and orderly.
  • Provides comfort, rest and activity for residents including transportation to other areas of the Ernest P. Barka Assisted Living Community.
  • Displays a friendly, caring, and warm approach while interacting with residents, staff and visitors.
  • Secures a safe environment for staff and residents by reacting to fire alarms and resident falls per facility policy. Redirects residents as needed.
  • Meets documentation standards by noting aspects of resident care in an accurate and timely manner, recording s vital signs: weight, height, and all pertinent information.
  • Assures resident safety and protects the rights and dignity of all residents at all times.
  • Maintains consistency of care by communicating actions, irregularities and needs of residents and in report and in orientation of new staff members.
  • Takes responsibility for own actions by using time efficiently to deliver high quality service to resident to demonstrate commitment to co-workers.
  • Identifies reports and describes violations of confidentiality or resident rights.
  • Identifies and utilizes infection control techniques at all times.
  • Identifies and reports violation of resident rights and suspected or witnessed abuse.
  • Identifies and supports spiritual belief systems of residents.
  • Encourages residents to participate in their care appropriate to the resident's optimal level of physical and mental functioning.
  • Contributes to team effort by volunteering to helping others to accomplish department and community goals in a caring positive manner to maintain the residents’ quality of care and quality of life.
  • Attends mandatory in-services trainings annually.
  • Must be able to work hours as scheduled based on the requirements of the position and department, including nights, weekends, and holiday shifts.

QUALIFICATIONS:

  • High School Diploma or GED; and
  • Two years related experience in a Health Care setting and/or training; or
  • An equivalent combination of education, work and experience will be considered.
  • CPR/Basic Life Support (BLS) Certified.
  • National Caregiver Certification Course (NCCC) welcomed, not required.
  • Licensed Nursing Assistant (LNA) License welcomed, not required.
