A reputable community hospital in warm, beautiful and coastal Northern California is actively looking for a Risk Management and Patient Safety Coordinator to add to its reputable team of professionals.
The Risk Management and Patient Safety Coordinator will work with a medical center that provides comprehensive healthcare services that effectively meet the needs of the surrounding community through compassionate and responsible care for over 50 years. The Risk Management and Patient Safety Coordinator will be part of a team that embraces family atmosphere, positive attitude to excel and to provide the best patient care.
The Risk Management and Patient Safety Coordinator will assist in the implementation of risk management and patient safety activities. The Risk Management and Patient Safety Coordinator will be responsible for review and analysis of event reports and data to identify trends and risk reduction and performance improvement opportunities. The Risk Management and Patient Safety Coordinator will assist in coordinating and monitoring action plans associated with risk mitigation activities, reporting of trends to departments and managers, identification of preventable versus non preventable outcomes, and facilitation of multidisciplinary teams to reduce the frequency of risk related incidents.
TheRisk Management and Patient Safety Coordinator will assist identification, evaluation and classification process related to safety events, participates in enterprise patient safety project initiatives and preparation of reports on risk/patient safety trends for the enterprise. The Risk Management and Patient Safety Coordinator will work collaboratively with internal teams to integrate, evaluate and implement the impact of information on the development of safe practices throughout the organization and achieving high reliability healthcare.
Reporting directly to the Director of Quality, Risk Management and Patient Safety, the Risk Management and Patient Safety Coordinator will provide technical support to staff about Performance Improvement principles. The Risk Management and Patient Safety Coordinator will be responsible for the timely collection of medical records, abstracting Core Measure Data into the Data Base, concurrent review of inpatients with core measure diagnoses, appealing any validation scores below 80, and designing action plans for improving scores. CPHRM certification preferred.
