PrimaryOne Health is currently seeking a Patient Resource Navigator. As the Patient Resource Navigator, you will help HIV and at-risk HIV negative patients get the support they need to access healthcare and health-impacting resources. You will gather information related to economic barriers, healthcare systems concerns, and basic needs (including, but not limited to, food, transportation, and material goods). You will also connect members directly to vetted community agencies and resources; may include community-based support. As the Patient Resource Navigator, you will also provide support and guidance to help members access Community Based Organizations and the PrimaryOne Health care systems.
Successful candidates will possess a high school diploma or equivalent. Interest in HIV and working with marginalized populations, strongly preferred.
Employment with PrimaryOne Health is contingent on new hires completing full COVID-19 vaccination within 45 days of start date to maintain employment (unless you request and receive an exemption).
