Schedule: Monday - Friday, 8:00am - 5:00pm

Duties and Responsibilities:

Our most successful Patient Navigator candidates will be energetic, engaging and deeply compassionate leaders who inspire their team to follow their lead. They will be organized, practical, proactive and responsive. They’ll be authentic and passionate, able to meet patients where they are, hearing them, and responding with grace and directness. They will be able to say what’s so while conveying compassion and concern, putting emphasis on the patient, letting them know that “we care about YOU.” They will have a high EQ, be both people and process savvy and be discerning while not judgmental. They’ll be people who flow easily with what others call stress, with being on call at a moment’s notice to meet people during an important time in their lives.

The Patient Navigator acts as a member of a multidisciplinary team to assess and coordinate patients’ care among multiple service providers during inpatient and outpatient treatment.

  • Assessing patient out of facility biopsychosocial stressors (i.e. housing, financial, legal, medical, employment, etc.) and developing a strategy to meet these needs during and after treatment episode
  • Facilitating multiple care aspects (case coordination, information sharing, aftercare planning, court related issues)
  • Maintains a good working relationship with support networks, government resources, community resources, employers, and referral sources.
  • Completion of all necessary documentation related to FMLA and Short-Term Disability
  • Maintains patient records by reviewing and completing case notes
  • Prepares patients’ discharge by reviewing and coordinating community services necessary to maintain sobriety outside of structured setting
  • Participates in weekly multidisciplinary treatment team as part of ongoing assessment of patient needs and plan for discharge
  • Daily involvement in census management including coordinating discharge schedule and communicating to treatment team and administrative team.
  • Ability to interact with patients in a calm and positive manner to understand all biopsychological stressors
  • Ability to communicate with various 3rd parties to obtain necessary approvals to remove patient’s biopsychological stressors (FMLA, STD, Court Dates, etc.)
  • Ability to respond to common inquiries or complaints from customers, regulatory agencies, or members of the business community.
  • Ability to write speeches and articles for publication that conform to prescribed style and format.
  • To take charge of patient’s out of facility biopsychological stressors and remediate effectively and efficiently
  • Effectively lead the discharge planning process so all documentation, surveys, and releases are completed
  • Competency with Microsoft Office Suite
  • Comfortable working with electronic health records

Requirements:

  • High School Diploma or equivalent.
  • Valid, unrestricted driver’s license
  • Clean driving record for at least 3 years
  • 1-year prior experience working with Substance Abuse populations strongly preferred
  • Previous Case Management experience strongly preferred
  • Must be committed to drug-free lifestyle and responsible alcohol use
  • Personal passion or interest in substance abuse issues strongly preferred

Additional Compensation includes but is not limited to the following:

  • Student loan repayment after 6 months of employment, Yearly max: $5,000.00.
