POSITION SUMMARY: The RN Patient Care Coordinator (PCC) works as an integral part of the care management team acting as an educator, resource and advocate for patients and their families. RN PCC is responsible for managing high risk patients with chronic and acute disease processes to provide effective guidance and education and promote goal directed plans of care to achieve patient centered positive outcomes. Responsibilities include coordinating patient care through the efficient use of resources and thereby enhancing quality and cost effective outcome. Acts as an advocate for individual’s healthcare needs, and coordinates care to minimize the potential fragmentation of care delivery. Part of team designed for promotion of excellent care effecting the patient and entire healthcare organization.
DUTIES AND RESPONSIBILITIES
  • Collaborate with patients and care teams to develop patient-centered care plans, with a focus on, disease management, prevention, and overall wellness.
  • Identify and engage appropriate patients for care management from lists and referrals, in collaboration with physicians and supervisors. Conveys patient findings with providers in timely manner to optimize provider visits.
  • Collaborate with community partners to facilitate patient and caregiver access to community resources relevant to the patient’s needs.
  • Performs accurate med reconciliation as needed.
  • Performs initial and periodic patient assessments. This assessment includes organized and pertinent collection of data about the patient with emphasis on healthcare needs and barriers to successful navigation of their healthcare. Focus on patient as a whole.
  • Assist in providing patient empowerment using motivational interviewing skills, problem solving and self-management goal setting.
  • Assist patients and/or caregivers in scheduling patient appointments and access services as needed.
  • Collaborate with community partners to facilitate patient and caregiver access to community resources relevant to the patient’s needs.
  • Documents concisely and accurately in EMR.
  • Attend required meetings, participate in committees, and assist with special projects as needed
  • Perform other duties as assigned
  • Remains current in changes in healthcare and seeks continuing education in care management/nurse coaching
EDUCATION & EXPERIENCE
  • Current NYS RN license required.
  • Experience as an RN in a physician office/hospital.
  • Current Infection Control Certificate
  • Basic Life Support (BLS) certification is required
QUALIFICATIONS & REQUIREMENTS
  • Excellent organizational skills required, with the ability to communicate and work collaboratively with all levels of staff, providers and office staff, and effectively utilizing all resources available.
  • Highly developed interpersonal skills including active listening and motivational interviewing experience.
  • Ability to assess, plan, implement and evaluate nursing care according to individual needs.
  • Familiarity with EMR systems preferred
