Paraprofessional/CSI: Clarke County Job at Pathways Transition Programs, Inc

Pathways Transition Programs, Inc Watkinsville, GA

-The Community Support Individual (CSI) staff will provide rehabilitative, environmental support and resource coordination considered essential to assist children, adolescents, and their families in accessing services and in creating environments that promote resiliency and support the emotional and functional growth and development of the youth served.

-Services are delivered utilizing a team approach and are usually provided in the home and within the family system. Some services may be offered in the school setting or via telehealth.

-Manage a part-time caseload (15 face-to-face with the client service hours weekly).

-Comply with all payor and PTP service documentation requirements using our state of the art, HIPAA-compliant, electronic medical record system (CareLogic).

-Attend supervision, clinical team meetings, and participate at regular agency trainings.

-Bachelor level or Master Level (unlicensed) to provide services in Bartow Counties or close surrounding area as needed.
