Connected Home Health and Hospice is a Medicare-certified, 5-star rated agency that is seeking an experienced and caring Occupational Therapist (OT) for our home health division in the Greater Portland, Oregon area. We are looking for someone just as passionate as we are about providing quality patient care.

Full-time with benefits + $1,000.00 sign-on bonus.

Therapy Services

· Assessment and evaluation

· Develop and implement Plan of Care and meet therapy goals

· Teach to help promote and maintain the patient’s optimal level of function and performance

· Cohesive and collaborative care with nursing

Requirements and Qualifications

· Occupational Therapist (OT) licensure in the state of Oregon

· Minimum of two years of experience as a OT

· Current BLS certification from AHA

· Be a licensed driver with an insured automobile in good working order

· Ability to comply with the State COVID Vaccination Mandate
