Primary Purpose of Position: The Community Health Worker’s (CHW) primary responsibility is to assist in the overall management of CCT patients, done through a population-based approach with the focus on individual interventions such as individual health and behavioral assessments to identify, prevent, and manage chronic illness with the focus to decrease inappropriate unnecessary utilization of healthcare. The CHW will provide support for the patient through their transitions of care and provide optimal linkage to community resources. In addition, the CHW will support patients by providing education/support on; but not limited to:

  • Nutrition/exercise
  • Disease/illness
  • Coordination of care
  • Social determinants (i.e. transportation, financial, housing, employment, etc.)
  • Bridging cultural gaps for minority populations

The CHW will be under the guidance of a “Clinical Leader” and will work collaboratively to improve the quality of patient care through the coordination of appropriate resources.

Education/Certification: High School Diploma required. Certification in Medical Terminology preferred.

Experience: The ideal candidate resides in the community they serve (downtown Lewiston/Auburn) and will have experience/knowledge regarding community resources in the L/A area. The candidate must be bilingual in English and Somali. Experience with motivational interviewing also preferred.

Skills: Must have the ability to work independently with limited supervision. Capable to make independent decisions regarding referrals and support to the patients and the appropriate notification to the CCT Care Manager as needed. Ability to maintain confidentiality.

Schedule of Employment: This is a part-time, 20-hour position. Temporary 1-year assignment with the opportunity to extend dependent upon grant funding.

Hourly Rate: $16.00/hour.
