About the Position
MHO Care Coordination Specialist will work in partnership with PacificSource Community Solutions Care Management team to manage behavioral health benefits for Lane County PacificSource Medicaid members. These CM MHO Care Coordinators support members in navigating the behavioral health care delivery system, review mental health and substance use disorder treatment records for care coordination and quality assurance, coordinate care directly with PacificSource members and providers, develop plans of care to meet the member's identified healthcare needs, and facilitate Interdisciplinary Care Team meetings. These daily job tasks require an ability to comfortably utilize a variety of electronic health record and data entry systems to document work completion and maintain member records.

This position requires a State of Oregon licensure as an: LCSW, LPC, LMFT, or PsyD.

This position is not a direct treatment provider position and there are no direct counseling tasks associated with this position.

In partnership with local Coordinated Care Organizations, LaneCare supports a range of mental health and substance use treatment services in Lane County including: services delivered by consumer and parent run organizations, community prevention efforts including parent training & suicide prevention for teens, crisis response, out-patient treatment, intensive community-based treatment supports and residential and hospital services.

About the Division
LaneCare is a division of Lane County Health & Human Services Department and is made up of three sub-divisions; LaneCare Administration, LaneCare Pacific Source Community Solutions, and LaneCare Trillium Community Health Plan. We are contracted by the two coordinated care organizations (CCOs) in Lane County to manage behavioral health services for Oregon Health Plan (OHP) members. Our staff can assist active OHP members in finding mental health counseling or treatment services and help coordinate care to ensure the best possible outcome.

Schedule: Monday - Friday; 8:00am - 5:00pm

QUALIFICATIONS:

  • Master's degree from an accredited college or university with major course work in a mental health field, psychology, sociology, counseling, or a related field.
  • Three years of responsible professional mental health experience, including experience with managed care demonstrating a comprehensive understanding of the system.
  • An equivalent combination of experience and training that will demonstrate the required knowledge and abilities is qualifying.

Special Requirements:

  • State of Oregon licensure as an: LCSW, LPC, LMFT, or PsyD required.

  • This position is subject to a full criminal offender information record check. If fingerprinting is required, the fee is paid for by the successful candidate and the expense is non-reimbursable.
  • Please note that as part of the screening process Lane County will verify license and certification status.
  • One of the essential functions of this position is to travel within Lane County (occasionally outside of Lane County) to completed scheduled meetings.

Pay: $29.33 - $39.16 per hour
