Medical Assistant/Care Manager

About the Agency: MedAssist empowers patients facing medical challenges to thrive by addressing their needs with comprehensive access to services through care management. Our Agency is expanding and we are looking to add some great people to our team!

Position Overview: As a Care Manager, you will be coordinating care for patients who are returning home from nursing homes or rehabilitation facilities. This job entails extensive communication with patients over the phone, therefore, being personable, compassionate, and understanding are important traits. We want to assist our patients with enhancing their health & overall well being by building lasting connections with them. This position reports to the Care Manager Supervisor.

General Responsibilities (including, but not limited to):

  • Researching & coordinating various services such as physical therapy, medical supplies, transportation, etc., in alignment with patients’ needs.
  • Extensively communicate with health providers, patients, key contacts, to ensure continuity of services.
  • Document patient notes in the electronic health record in an accurate, timely, and detailed manner for approximately 50 patients per month.
  • Follow-up on phone calls, emails, and correspondence promptly to ensure quality standards of service delivery are met.
  • Perform other duties as assigned.

Job Type: Full-time

Pay: $18.00 - $20.00 per hour
