Position summary:
Encore ADHC is an Adult Day Health Center and Attendant Care provider serving a variety of counties across Kentucky and Indiana. Our agency is committed to serving the Elderly Adult community by providing an enriching day center experience as well as in-home health service. Encore ADHC specifically strives to cater to Lexington’s international communities by providing services designed specific for each community. We are currently seeking to hire a Community Coordinator to join our Community Outreach and Development department. This individual will work in close proximity with other Community Coordinators and administrators. This is a field position aimed at identifying individuals that might benefit from program services and then assist with their case.
Qualification
- Outgoing, and Friendly personality
- Willingness to learn about the Medicaid Waiver that allows eligible clients to receive services through their insurance
- Passion for serving the elderly community
- Ability to communicate clearly and effectively in Arabic
- Presentable and professional appearance
- Effective time management skills; able to manage multiple meetings and client appointments promptly
- Goals-driven individual that is able to share our services and successfully connect Encore to clients in need of help

-BILINGUAL

Responsibilities:
-source new leads for potential clients for the Day Center and Attendant Care (in-home department)
-client intake (assisting with all aspects of the process when a client starts the application process until the process is finalized, and the client start’s attending)
-application coordination (facilitating and attending State Medicaid assessments, submitting requested documentation to the local Medicaid and SSI office)
-effectively contribute and reach department goals
-participate in productive weekly team meetings
-communicate for daily progress updates with team
-sourcing new leads for potential Attendant Care (in-home health) clients
-case manager coordination (communicating and working closely with local case managers to generate appropriate Plan of Cares)
This individual must be a distinguished problem solver and multi-tasker; someone that is able to wear many multiple hats able to accomplish goals for the given day. Other personal and professional attributes include:

Job Type: Full-time

Pay: $17.00 - $18.00 per hour
