LVN Case Manager/ QA Job at Choice Health Group
Choice Health Care Group is seeking immediate hire for an LVN Case Manager to join our growing team. The LVN will monitor the plan of care to reflect admission database, on-going findings, age-appropriate care, cultural specific needs, and appropriate acuity.
LVN Case Manager Responsibilities:
- Leads effective development and completion of discharge plans including discharge barriers and patient/family education.
- Discusses w/ patient, family/significant others about the plan of care.
- Monitors the patient's progress based on their care plan.
- Revises plan on an ongoing basis based on patient condition and evaluation of progress in coordination with the clinical team.
- Answers patient questions about medication purpose, side effects, and administration instructions.

LVN Case Manager Qualifications
- Graduate of an accredited practical nurse or vocational nursing program.
- One to Two year minimum clinical case management experience. Community health/hospice or medical/surgical experience is preferred.
- Currently licensed as an LPN/LVN in the State.
- Complies with accepted professional standards and practice.
- Demonstrates good verbal and written communication, and organization skills.
